Job Summary
This position reports to the Clinical Manager or designee, the RN will work with diverse populations with multifaceted needs. The incumbent will serve as a care planner, care coordinator, advocate and record keeper for the patients of THC and their families. This individual will also be responsible for providing nursing care through, nursing assessment, teaching, nursing plans of care and routine nursing procedures. The nurse-patient/family relationships, continuity of care and accountability are central to the delivery of individualized high quality patient care. This position works in collaboration with the physician, other healthcare providers, and other ambulatory support staff within the department and with other disciplines to plan and coordinate the delivery of excellent patient care.
Essential Functions
Responsible for care in collaboration with the patient’s physician, the patient and/or family representative, and other members of the clinical team.
Accountable for the clinic providing safe patient care and demonstrating organizational skills that maintain and coordinate safe delivery of quality care. Maintaining logs, checking expiration dates of medications and medical surgical supplies, maintaining emergency cart and ensuring compliance with the Maryland VFC Program.
Demonstrates the knowledge and skills necessary to provide expected standards of care based on physical, psychosocial, cultural, educational, spiritual, safety and age-appropriate considerations of patients.
Provide quality care and customer service to the patients and their families, regardless of their socio-economic status. Demonstrate autonomy by utilizing the nursing process to assess patients and determine next steps in the delivery of care.
The staff RN shall bear full and ultimate responsibility for the quality of nursing care he/she provides to individuals and groups, including but not limited to health maintenance, teaching, counseling, collaborative planning and restoration of optimal functioning and comfort.

Responsible for developing, implementing and evaluating individualized patient care plans and facilitates coordination of care across the continuum.
Practices in accordance to Total Health Care policies, protocols, procedures and workflows.
Maintain a current level of nursing knowledge and skills by completing all mandatory Health Stream Modules and scheduled competencies.
Maintain compliance with infection control procedures and regulatory requirements
Maintains and uphold all regulatory requirements and Patient Centered Medical Home requirements.
Accurately performs medication administration, venipuncture, clinical nursing assessment/triage, care planning and other clinical nursing skills as required.
Responsible for delegation and supervision in accordance with the Nurse Practice Act – Provide leadership and oversight of the health care team to ensure efficient, quality care is provided by the members of the healthcare team.
Communicates regularly and as needed with Clinical Manager, keeps manager aware of any concerns in the clinic.
Act as a patient advocate and serves as a liaison between patients, their families and healthcare providers and health care team to improve patients’ quality of life through collaboration and follow up care.
Providing excellent customer service to internal and external customers, patients and their families under all circumstances, exercising conflict resolution skills with customers and staff when necessary.
Document thoroughly and appropriately in the EMR or other reports as required. ie nurse visits and triage/walk ins if necessary
Assist with timely completion of forms and documents needed by patients, families or other required documents
Assures timely and efficient flow of the clinic – Prioritizes tasks efficiently and effectively and completes Patient Cases and Follow Up in a timely fashion according to THC policy.
Maintain HIPAA regulations
Ensure medical care and follow up are planned and documented and that all requests for services are completed for each patient upon exit.
Attends staff and department meetings as needed
Performs other duties as assigned
Minimum Education, Training & Experience Required
Requirements include a current Maryland RN licensure, graduate of an accredited nursing program and one year minimum experience in an acute care facility required, BSN preferred. Current BLS certification.
Required Knowledge, Skills & Abilities
Must possess excellent physical assessment, communication and critical thinking skills. Excellent customer service is a must. Ability to resolve complex customer service issues and exercise conflict management, proficient in EMR documentation, excellent follow through skills prioritization of multiple tasks and attention to detail required. Some weekend and evening hours are required. Ability to work in a fast-pace environment. Excellent written, verbal and interpersonal communication skills, as well as the ability to multi-task and successfully work in a team environment. Ability to perform assigned duties with a high degree of accuracy.
