POSITION PROFILE:

Joining our team as a Case Manager may be your next step! The Case Manager works in the Healthy Start program, a school-based family resource center at Field Elementary School. A student’s educational success, physical health, emotional support, and family strength are inseparable. The Healthy Start Program prioritizes specific services centered around social, emotional, physical, and mental wellness for students and their families. Case Managers use independent judgment, creativity, and adaptability to coordinate intensive client services. The ideal candidate will have experience providing direct client and family support services, group facilitation, and family/community program coordination.
This position is 100% out stationed on campus and is not eligible for remote work options.
ESSENTIAL JOB FUNCTIONS AND RESPONSIBILITIES:
- Facilitate socio-emotional groups for high school students.
- Provide one-on-one socio-emotional support utilizing Cognitive Behavioral Therapy-based curriculum.
- Support the outreach, recruitment, and retention of program participants.
- Provide care coordination and case management services maintaining accurate documentation of client files as well as accurate and timely entries into a client tracking database.
- Develop and maintain positive relationships with students, families, school staff, and community organizations,
- Collaborate with multiple stakeholders to coordinate, plan and implement family and community enrichment and education programming
- Attend relevant community, school site and district meetings.
- Other tasks as assigned
QUALIFICATIONS (Education, Experience and Certifications):
- Bachelor’s Degree in Social Work, Child and Youth Development, Humans Services, or related field preferred
- Experience working with diverse populations with varying backgrounds.
- Experience working with school systems is highly desirable.
- Experience in direct client service, particularly experience with youth and families, and community development
- Experience facilitating groups, case management, and delivering curriculum-based interventions.
- Fully vaccinated against COVID-19 at time of hire
KNOWLEDGE, SKILLS AND ABILITIES:
- Computer literate & proficiency of Microsoft Office package
- Ability to take initiative, be self-directed, and work independently.
- Ability to manage multiple demands and prioritize when needed.
- Strong customer service skills as well as excellent verbal and written communication skills
- Ability to work some evenings, as needed.
- Bilingual in English/Spanish is preferred, but not required.
- Access to reliable transportation and willing to use it for work-related purposes.
