TITLE: Physical Therapist Outpatient / Physical Therapist Clinical Specialist Outpatient

JOB OVERVIEW: The Physical Therapist position requires current Physical Therapy skills to evaluate, plan, implement and/or delegate a comprehensive plan of patient care in the orthopedic, pre/post-surgical, neurological, and health/wellness service areas.
DEPARTMENT: Outpatient Therapy or Lifestyle Medicine
WORK HOURS: 7:00am - 7:00pm Monday - Friday (varies by clinic)
8- or 10-hour shifts may be available, dependent upon patient/clinic need
REPORTS TO: Manager, designee - Supervisor or Lead
PREREQUISITES:
Current or in process Washington State PT License or Interim License
QUALIFICATIONS:

UNIQUE PHYSICAL/MENTAL DEMANDS, ENVIRONMENT AND WORKING CONDITIONS: (See Generic Job Description for typical physical/mental demands, environment and working conditions):
Occasionally requires transfer of patients weighing up to or exceeding 200 lbs. needing maximum assistance from wheelchair to table and back to chair. This position requires the ability to recognize own limitations with regards to load handling and regularly seeks assistance when demand of transfer or lift exceeds individual physical abilities. This position requires standing, sitting, crouching, bending, and walking for extensive periods of time. This position requires hand dexterity for manual treatment techniques and to operate equipment (i.e., ROM, manual therapy treatments, ultrasound, or electrical stimulation unit, etc.). Must be able to lift, carry a minimum of 40 pounds to complete patient care responsibilities.

Maintains all appropriate certifications and licenses specific to the role.

PROVISION OF CARE, TREATMENT AND SERVICES - see Generic Job Description - Clinical Partner
Perform evaluations that include patient/client functional status appropriate to the patient/client diagnosis, needs or condition.
Collect, analyze, and integrate the information in the evaluations from all sources of the health care team including sources such as but not limited to; patient/family interview, patient observation, RN, MD, and other discipline specific assessments. Ability to integrate this information to set appropriate plan of care for therapy services.
Elicits information from patient/client on the identified needs and priorities and incorporate this into the assessment and plan of care.
Set specific goals in the plan of care as determined at the time of the evaluation. Specific goals will assist each patient/client in restoring, improving, or maintaining each patient/client optimal level performance in self-care, self-responsibility, independence, and quality of life.
Adheres to the Department and Organizational Policies/Procedures and Treatment Protocols in all clinical areas.
Perform and document re-evaluations or re-assessments with the revision of treatment plans and goals according to standards and at regular intervals. This is dependent upon the patient/client significant change in condition or diagnosis, or per standards of care intervals.
Perform telehealth/virtual visits using appropriate documentation and coding standards of care.
Provide patient/client education that will assist them in meeting their goals and increase their functional skills and improve their quality of life and health.
Act as a resource for the development, refinement, and revision of patient educational materials for quality service delivery.
Collaborates with the full care team to provide appropriate patient care information and alters treatment plan as needed by incorporating information obtained by other team members into the treatment plan.
Ability to determine the need for further treatment, discharge, or continuation of care with appropriate services including prescribed fitness. Ability to provide appropriate information to subsequent providers related to the patient/client's plan of care and services when a patient moves to different services.
COLLABORATION AND MENTORSHIP
Provides direct supervision to Physical Therapy Assistant, Exercise Specialist, and Rehab Aide staff for patient/client treatment and programs.
Delegates patients appropriately to assistant and exercise staff with clear communication and guidance as needed for successful patient care outcomes.
Provides answers to the Assistant Therapist/Exercise Specialist questions and provides feedback and education for treatment techniques and treatment progression.
Provides clear and concise information to support staff for accurate assistance in patient scheduling, registration and financial information.
Orients new staff, students, and volunteers as appropriate.
INFORMATION MANAGEMENT
Completes authorization or certification for care in a timely manner to allow for continuity of care for outpatient physical therapy.
Assists in obtaining orders and written prescriptions for appropriate therapy. Follows up as necessary in accordance with developed standards.
Perform accurate, appropriate, and timely documentation according to the standards, protocols, and pathways as appropriate. Completes documentation within 3 days of the completed visit or before next scheduled visit for therapy services.
Follows policies and procedures for appropriate billing in a timely manner for patient care.
Understands and properly utilizes Current Procedural Terminology (CPT) codes in accordance with documented services performed during treatment. Demonstrates an understanding of the 8-minute rule' vs. substantial portion methodology' to appropriately apply coding for services performed. Understands and incorporates all other rules, laws and regulations pertaining to patient billing and documentation for quality patient care.
Utilizes technology for communication with team as appropriate. Utilize email, internet, voicemail, and other technology to create an efficient environment for teamwork.
Ability to utilize all technology to assist with meeting patient care needs, quality management reviews, and outcome information and improving quality patient care outcomes. Able to utilize technology in monitoring patient information, quality improvement processes, and program evaluation.

LIFESTYLE MEDICINE CORE COMPETENCIES (Lifestyle Medicine Department)
The Physical Therapist in Lifestyle Medicine has the unique requirement of working in an interdisciplinary care team to provide comprehensive lifestyle modification and behavior change programming. They have specific knowledge and understanding of the lifestyle medicine field and the physical therapists' role in exercise prescription and active rehabilitation. As a member of the Lifestyle Medicine clinical team, this position requires competency in health risk assessment measurements and assessment of values, with incorporation of these results into an integrated plan of care.
Lifestyle Medicine-Specific Patient Care Competencies
Independent performance and assessment of Health Risk Stratification to be incorporated into the patient assessment and plan of care design.
Collaborates with the Lifestyle Medicine care team, including the Exercise Specialist and Registered Dietitian, to further assess patients' total health needs.
Incorporates a comprehensive health plan with long-term goal setting as indicated.
Incorporates motivational interviewing skills to partner with patients for care planning and long-term goal setting.

Clinical Specialist Prerequisites:
1. All Prerequisites required for the Outpatient Physical Therapist
At least two years of current clinical practice experience
Has completed and maintains department approved advanced certification
Ability to prioritize tasks for patient care and projects to meet establish standards and deadlines
Clinical Specialist Specific Job Duties:
In addition to the job responsibilities outlined below for the Outpatient Physical Therapist, the Clinical Specialist will participate in formal mentorship, onboarding, or clinical instruction in one or more of the following ways:
Provide peer mentoring to new and/or experienced therapists for a particular set of evaluation and/or treatment skills.
Provide direct supervision to volunteer/job shadow person for department. Takes responsibility for volunteer supervision with task development, training, and assessment of services - revises tasks as necessary.
Provide direction and clinical supervision to student for patient observation, evaluation, treatment, and other professional experiences for the department.
Takes responsibility for student supervision with task development, training and patient assessment and delivery of all services provided by the student. Will also revise the program for specific student program as necessary.
