Position Summary:
The RN Care Manager is key role that drives the success of our Medication Assistant Treatment (MAT) program in our Primary Health Care Division. As the main liaison for MAT, the RN Care Manager will lead the team which includes licensed social worker and peer advocate personnel. The RN Care Manager will ensure that the MAT program is effective in treating patients with substance abuse disorder, with an end goal of relapse prevention. Duties will include ensuring compliance with federal, state, and organizational guidelines and procedures.
Responsibilities:
- Provides oversight of buprenorphine/naloxone intake assessment, induction, stabilization, maintenance, and relapse management.
- Ensures that state and federal guidelines are followed and maintained.
- Collaborates with providers, social worker/counselors, psychiatrists, pharmacy, primary care physician, and specialty care physicians to whom the patient has been referred
- Coordinates between provider and pharmacy, including obtaining medication history, assisting with prescription processing and refills, prior authorizations, insurance coverage, and preferred medication formulary
- Upholds procedures related to diversion, use, safety, and storage of buprenorphine
- Refers patients for services in Behavioral health, Health Homes, and Chemical Dependence Outpatient Programs
- Conducts intake for all new patients and enrolls into the Medicated Assisted Treatment Program (MAT)
- Supports medication management, creates plans of care, and researches treatment options.
- Serves as a professional patient advocate and promotes the welfare of the patient.
- Serves as a liaison between patients, families, and healthcare providers
- Employs screening and assessment tools to help identify significant uncovered risks and problems with drug and alcohol use.
- Assists prescribing provider in:
- Ruling out comorbid acute or chronic pain disorders and opioid dependence
- Ruling out polysubstance use
- Ruling out co-occurring psychiatric disorders
- Questioning about potential pregnancy and child-bearing status
- Screening for infectious diseases [hepatitis viruses, HIV, tuberculosis (TB), sexually transmitted diseases (STDs)].
- Screening out domestic violence or abuse
- Other duties that may be assigned.
Requirements:
- Bachelor’s Degree in Nursing
- NYS Licensed Registered Professional Nurse
- At least 3 years of direct nursing experience working with vulnerable populations (i.e. behavioral health, psychiatric, etc.)
- Bilingual Spanish/English required
- Familiarity with Subaxone Medication Management
