Weingart Center, a 501(c)(3) nonprofit agency with multiple locations throughout the Downtown Los Angeles area is hiring for a Case Manager.
Under the direction of the Program Manager, the Case Manager provides high quality wrap-around services and is responsible for the overall documentation and maintenance of client records, meeting with clients on a weekly basis to check on progress, and re-evaluate case plans as necessary utilizing the Housing First, Harm Reduction and Trauma Informed Care modalities under Weingart Center's Interim Supportive Housing Program.
Weingart Center's programs provides safe and secure shelter, meals, case management and mental health services on a 24-hour basis for adults experiencing homelessness. Our multiple sites throughout the Los Angeles area incorporates low barrier, trauma-informed care, and harm reduction policies and procedures into their program design and housing operations.
This is an on-site, in office position.
WHAT WE LOOK FOR:
- You have a strong working knowledge and case management experience working with the unhoused population, adult social services, the welfare system, and legal, corrections or health care systems.
- You have experience in substance abuse, alcoholism, mental health care, and other systems associated with this targeted, formerly homeless population.
- You have working experience in case management databases such as HMIS (Clarity) and/or ICMS (CHAMP).
- You have knowledge and work experience with government agencies such as LAHSA, DHS and DMH.
- You have strong communication and writing skills.
- You have a strong commitment to advocate effectively on behalf of low-income and/or formerly homeless clients.
- You have empathetic and have excellent listening skills.
- You have a Bachelor or Associate degree in Social Work, Psychology or equivalent field, preferred.
- Bilingual fluency in English and Spanish, a plus.
Salary starts at $21.00 per hour, commensurate upon combined and related experience, educational level and bilingual fluency.
Weingart Center no longer requires (though highly recommends) position incumbent to be fully vaccinated against COVID-19 as defined and mandated by the Centers for Disease Control (CDC), the California Department of Public Health (CDPH), the Department of Health Services of Los Angeles County (DHS), and the California Occupational Safety and Health Act of 1973 (Cal/OSHA) effective August 4, 2023. However, this mandate may be reinstated at any time as recommended by these governing agencies.
