Job summary
The Bilingual Care Coordinator (CC) is a critical member of the clinical care team. The Bilingual care coordinator is office-based and responsible for providing administrative support to the clinical team as they provide cost-effective, high-quality health care for patients. Responsible to the clinical team and reports to the Care Coordination Supervisor.

Provide administrative support to the clinical care team

Answers incoming phone calls for general inquiries and redirect as appropriate

Route incoming faxes and assign as appropriate

Assist with the implementation or finalization of patient orders that do not require an RN to complete

Facilitate medical records management for outside documents that are critical for patient management, i.e. laboratory results, discharge summaries, medical documentation from specialists and primary care provider

Support scheduling appointments and reminder calls or texts
- Assist with referrals and referral management

Assist with follow-up to assure needed DME is ordered and delivered

Support a variety of data entry functions

Provide oversight for all Care Coordination functions including but not limited to orders, referrals, disenrollment, scheduling, error processing, medical records and consent uploading, mail retrieval and sorting, email processing, etc.

Complete other duties that support the clinical care team as assigned

Minimum of a High School diploma

Exceptional verbal and written Communication skills

Understanding of medical terminology

Completion of a Medical Terminology course preferred

Medical office experience preferred

Strong work history

Working knowledge of computers and ability to manage data within an electronic system

Five years of work experience preferred

Able to be on the phones throughout the day
- Must be fluent and able to read in both English and Spanish

Must be a resident of Puerto Rico
