OVERVIEW:
We are seeking dedicated Registered Nurses to join our growing home health agency in Marin County, CA, to provide skilled nursing services to patients in their homes. The ideal candidate will have a passion for providing high-quality patient care and possess a diverse skill set in various medical specialties, especially wound care, valid driver’s license, auto insurance, and ability to commute to patients' homes in Marin and nearby Counties.
We have multiple positions open for per-diem, part-time, and/or full-time RNs with great benefits such as 401k/529 with match, health/dental/vision insurance for full-time, mileage reimbursement, travel reimbursement, cell phone reimbursement and/or provided tablet. Future opportunities to move to full-time.
QUALIFICATIONS:
1. Graduate of an approved school of professional nursing and currently licensed in California.
2. Two (2) years' nursing experience, preferred, but at least one (1) year required.
3. At least one (1) year experience in home health strongly preferred.
4. Additional certifications, such as wound care, preferred.
5. Russian language proficiency preferred, but not required.
6. Acceptance of philosophy and goals of this Agency.
7. Ability to exercise initiative and independent judgment.
RESPONSIBILITIES:
1. Provides skilled nursing services in accordance with physician orders and plans of care.
2. Sometimes, makes the initial evaluation visit and regularly reevaluates patients' skilled nursing needs.
3. Initiates appropriate preventive and rehabilitative nursing procedures.
4. Prepares clinical and progress notes for each patient visit and summaries of care conferences in a timely manner as per Agency policy.
5. Coordinates services with other Skilled Nurses, Physical Therapists, Occupational Therapists, Speech Therapists, Medical Social Workers, and/or Home Health Aides as needed.
6. Informs the Agency of changes in the condition and needs of the patient.
7. Counsels the patient and family/significant others in meeting nursing and related needs.
8. Participates in and presents Inservice programs.
9. Understands and adheres to established Agency policies and procedures.
10. Creates and/or signs orders and notifies physicians of patient needs and changes in condition.
11. Sometimes completes certification/recertification orders and discharge summaries.
12. May supervise and teach other nursing personnel.
13. Participates in peer review and Quality Assessment and Performance Improvement as assigned.
14. Gives total patient care as needed.
15. Takes on-call duty nights, weekends and holidays, as assigned.
16. Sometimes completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames.
17. Appropriately utilizes ICD-10 codes.
WORKING ENVIRONMENT:
Works indoors in Agency offices and patient homes and travels to/from patient homes.
