BronxCare Health System, a major teaching hospital system, now has an excellent opportunity for a Certified Diabetes Educator to provide self-management education and medical nutrition therapy for patients diagnosed with diabetes in pregnancy. Additional responsibilities of this position include documenting all visits in the Electronic Health Record and the outcomes database, collaborating with other members of the interdisciplinary care team to develop person-centered diabetes plans, supporting technology-enabled diabetes education and care, working with community partners to establish and maintain support options, and participating in community outreach efforts.
Qualified candidates must have a Bachelor’s degree in dietetics/nutrition, Registered Dietician/Nutritionist (RD/N) credential, current New York State Certified Dietitian Nutritionist (CDN) credential, and Certified Diabetes Care and Education Specialist (CDCES) or Board Certified-Advanced Diabetes Management (BC-ADM) credentials.
