The Mission of JWCH Institute is to improve the health status and well-being of under-served segments of the population of Los Angeles Area through the direct provision or coordination of health care, health education, services, and research.
Under the supervision of the Program Manager, the Referral Coordinator is primarily responsible for the coordination and maintenance of the health care center’s referral system. The Referral Coordinator also acts as liaison between the clinic, medical facilities, and other social service agencies to coordinate health services for JWCH's patients.
Principal Responsibilities:

Ensure all referrals are complete, accurate and processed in a timely manner which will include the faxing or mailing of referral request and necessary paperwork.

Schedule appointments and verify that the patient receives the care the primary care provider has requested.

Act as a liaison between the primary care provider, patients, specialists, and health plans.

Follow-up with patients, including reminding them of referral appointments.

Notifying primary care provider if patient does not follow through with referral appointment.

Maintain an environment conducive to patient comfort and confidentiality.

Provide clerical support for the clinic, including: providing patients with financial assistance and other resource information, paperwork, documentation in medical record.

Provide support and assistance to patients and their families regarding referral-related inquiries and concerns.

Stay updated on referral guidelines, insurance requirements, and healthcare regulations to ensure compliance.

Performs other clerical tasks as assigned.
Requirements:

High school diploma or equivalent; associate or bachelor's degree in healthcare administration or related field preferred.

Proven experience working in a healthcare setting, preferably in a role involving patient referrals or coordination.

Proficiency in using electronic medical records (EMR) systems and other relevant software applications.

Strong organizational skills with the ability to prioritize tasks and manage time effectively.

Excellent communication and interpersonal skills, with the ability to interact professionally with healthcare providers, patients, and team members.

Attention to detail and accuracy in data entry and record-keeping.

Knowledge of medical terminology, insurance procedures, and referral processes.

Ability to maintain patient confidentiality and adhere to HIPAA regulations.

Adaptability and flexibility to handle changing priorities and work independently or as part of a team.

Strong computer skills including MS Word and Excel.

Able to work with people from diverse ethnic, cultural, and socioeconomic backgrounds.

Bilingual English/Spanish preferred.
