Roles & Responsibilities:
Care Coordination is a program based on the case management model that assists patients in accessing HIV care, communicating with providers, obtaining needed social services, and initiating or adhering to antiretroviral treatment while providing education and capacity building to help patients become self-sufficient.
The Community Health Advocate is responsible for the day-to-day medical and social services support, treatment adherence and health education of patients enrolled in the Care Coordination program. The Community Health Advocate is part of an interdisciplinary team that includes medical providers, social workers and Patient Care Coordinators.
This is a full time, field-based position. Community Health Advocates are expected to spend 60% of their time in the field completing home- and field-based visits with patients throughout all five boroughs.
Job responsibilities include:
-Carrying out the tasks needed to execute patients' comprehensive treatment plans;
-Conducting monthly medication adherences assessments and biannual reassessments;
-Providing field-based health education and skill building to patients on a monthly, weekly or daily basis, depending on each patient’s level of need;
-Coordinating ongoing navigation and logistical support to help patients attend their health care and social services appointments;
-Accompanying patients to primary care, specialty care, and social service appointments as needed;
-Identifying patient needs and providing referrals to appropriate specialty care and community resources;
-Tracking each patient’s progress over time and communicating changes to the rest of the patient’s care team;
-Participating in interdisciplinary collaboration;
-Maintaining appropriate documentation as required by the hospital and the program funder;
-Performing data collection and data entry as required by the program funder;
-Other tasks as required.
Requirements:
-High school diploma or equivalent education or work experience
-Interest in working with a high-needs HIV+ patient population
-Ability to travel throughout New York City by public transportation and comfort with conducting home visits in all five boroughs
-Computer skills
-Exceptional interpersonal skills and cultural competency
-High level of organization and attention to detail
-Ability to independently manage a complex work schedule

Compensation
The Mount Sinai Health System (MSHS) provides a salary range to comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $22.64 - $23.89 Hourly. Actual salaries depend on a variety of factors, including experience, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
