Summary:Community Center seeks dedicated, outgoing social worker to provide older adults with targeted referral to community resources, develop and implement meaningful programming, and bolster community relationships with other service providers and referral sources. Candidate should possess strong networking and outreach abilities, assessment skills, knowledge of community resources, and program facilitation skills. Must be a team player. Prior experience in a geriatric social work setting preferred.
Key areas of responsibility:
· Conduct community outreach
· Provide information and referral to older adults and family caregivers reaching out to the agency for support and resources
· Follow up as appropriate to make sure that older adults successfully connect with needed resources
· Conduct pre and post surveys to document impact of interventions on isolation and loneliness
· Document service provision and collaborate with Director of Senior of Adult Services on reporting to funders
· Conduct intake assessments for new clients for the agency’s Parkinson’s and Memory Care programs
· Coordinate all aspects of agency’s weekly Pathways for Parkinson’s program, including but not limited to:
· Scheduling of guest speakers and fitness instructors
· Working with food service provider on program menu
· Creation and distribution of bimonthly program calendar
· Communication with other program staff
· Facilitation of program sessions
· Follow up with individual program members and caregivers on an as needed basis
· Implement new Social Circle program for older adults seeking entertainment and social connection
· Implement various social and entertainment programs for older adults
· Facilitate aging-related support and discussion groups
· Provide fill-in staffing of Memory Care programming on an as-needed basis
Supervisory Responsibility:
- Task supervision of staff within the Parkinson’s program and task supervision of social work interns.
Position Type/Expected Hours of Work
Full time 40 hours/week. Ability to flex hours to staff evening programming on an as-needed basis.
Required Education and Experience
Social Work Masters with LMSW
2-3 years post-Masters experience, preferred
Knowledge and Skills:
· Must be organized and detail orientated.
· Basic overall knowledge of Microsoft Office software with proficiency demonstrated in specific programs deemed critical to achieving performance goals.
· Excellent writing and verbal communication skills.
· Background in government benefits and entitlements.
· Familiarity with working with clients with mental health issues and interacting with the mental health system on clients’ behalf
· Evidenced commitment to an interdepartmental, interdisciplinary team approach to work; strong conflict resolution and collaboration skills.
· Creative, energetic, positive demeanor a must.

Pay: $22.50 - $25.00 per hour
