Responsibilities:
- Develop and implement individualized care plans for individuals with intellectual disabilities
- Provide direct care and support to individuals, ensuring their physical and emotional well-being
- Monitor and document individual progress, behaviors, and medical conditions
- Collaborate with interdisciplinary teams to assess and address the needs of individuals
- Conduct assessments and evaluations to determine appropriate interventions and services
- Provide behavior management and crisis intervention as needed
- Ensure compliance with HIPAA regulations and maintain confidentiality of sensitive information
Experience:
- Minimum of [X] years of experience working with individuals with intellectual disabilities
- Knowledge of medical terminology related to intellectual disabilities and associated conditions
- Strong conflict management skills to effectively handle challenging situations
- Familiarity with senior care, dementia care, hospice care, and related systems
- Ability to develop and implement care plans tailored to individual needs
- Proficient in documenting vital signs and other relevant information
