Description:
The Health Information Technician (HIT) is responsible for ensuring all medical records are collected, processed, and maintained as defined by hospital policy and procedure and in compliance with regulatory and accrediting agency rules and guidelines. Responsibilities include working in electronic health records, scanning documents into the EHR, and auditing medical records as needed. This position will also be required to enter and gather information for reports run from various computer systems used to support HIM department goals.
ESSENTIAL FUNCTIONS
Ensures that all medical records are collected, processed and maintained as defined by health system procedure Records are collected within defined time frame post discharge or visit

Records are checked to ensure that all components are present at the time of receipt or collection

Missing records and record components are sought immediately and an escalation process followed to ensure a complete record is received

Assembles/Preps Records into defined order

Checks the record thoroughly to ensure that correct patient and account information are present

Maintains data related to missing and incomplete records as established by policy
Prepares, scans, verifies, and index paper documents into the EHR

As assigned, performs scanning, verifying and indexing duties with accuracy and efficiency

Performs quality checks of medial record imaging and prepares statistics to support record integrity
Ability to analyze records for completeness and assign deficiencies in incomplete record database

Following policy and procedures analyzes records for key documentation elements and accurately assigns Provider deficiencies in a timely manner
Process and compile statistics for delinquency

Runs reports from chart deficiency module to determine delinquent record count

Notifies Supervisor of Provider chart completion activities

Collects, maintains and reports Provider Suspension data
Retrieve electronic medical records for internal and external requests as appropriate

Ensures that all appropriate record components are retrieved for internal and outside auditors

Ensures that time sensitive requests are addressed within time frames defined

Ensure release of information is completed according to policy
Performs medical record audits ,as needed, to support hospital Quality Department activities Maintains a working knowledge of applicable Federal, State and local laws and regulations, Trinity Health Corporate Integrity Program, Code of Ethics, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical, and professional behavior. Adheres to all Mercy Health System Policies and Procedures. Maintains medical records operations by following policies and procedures; reporting needed changes Demonstrates knowledge and compliance with the Hospital Safety and Infection Control policies as required by OSHA and the Patient Safety Program.
