Under the direction for the supervising social worker, the clinical social worker will engage with individuals living with HIV (or affected by HIV) who are unsheltered or experiencing unstable housing conditions. This dynamic role involves providing comprehensive social work services within a drop-in primary care clinic known as Positive-health Program for Unstably housing Populations (POP-UP) and Health Access Point (HAP). The clinical social worker will establish meaningful connections with homeless individuals and their families with the purpose of securing stable housing by offering social service referrals and facilitating access to clinical services and relevant community resources. You will play a crucial role in assisting individuals and families throughout the housing application process, ensuring the acquisition of necessary documentation, and establishing benefits for successful housing placement. The clinical social worker will provide compassionate and supportive interventions to address psychosocial issues, enhance overall functioning, and help individuals develop insights that reduce stressors and distress. Your support will extend beyond securing housing, as you will also assist those who are already housed in adjusting to independent living and accessing community resources essential for maintaining housing stability. The clinical social worker will provide basic education about antiretroviral treatments, supporting individuals in initiating and maintaining consistent adherence. By reducing transmission risks and minimizing medical complications associated with HIV, you will make a significant impact on improving overall health outcomes.
Collaboration is key in this role, as you will work closely with an interdisciplinary healthcare team and contracted agencies to support patient stabilization and ensure access to vital supportive services. Your active involvement in weekly case conferences, staff meetings, in-service trainings, and other mandatory clinical meetings will foster a cohesive and informed approach to care delivery.
