The Case Manager is responsible for providing advocacy, information and referrals for participants within the DFC homeless service programs. The typical working hours for these positions range from 8:00 am-9:30 p.m. Full-time and part-time (evenings) positions available.
RESPONSIBILITIES
- Set up and break down program operations
- Conduct intake and assessments for new participants
- Develop service plan with a focus on financial stability and re-housing
- Provide individualized resources and referrals to community programs
- Assist with clients in advocating for services for which they qualify
- Provide employment development support
- Work with homeless service team for client re-housing
- Develop partnerships with other community-based organizations
- Outcomes and data management into MHIS for participants activities and accomplishments
- Follow-up with participants who have exited the program
- Meet bi-weekly with supervisor for case review
- Other duties as assigned

Pay: $21.00 - $24.00 per hour
