This is a full-time position to provide home and community-based services to individuals and families.
EDUCATION: Minimum of a Bachelor’s Degree in a human service related field.
EXPERIENCE: Minimum of one (1) years’ experience working with children/
adolescents. Familiarity with youth behavior issues, juvenile
justice system, trauma-based approaches to clients, wraparound training and coordination of resources.
SKILLS: Communication – Group facilitation, good organization and writing skills.
DUTIES/RESPONSIBILITIES:
1. Provide case management to Clients.
2. Receive referrals, assure case management reports are complete and maintain Client files.
3. Provide in-home mentoring, role modeling, and out-of-home activities (i.e. job searching,
recreation, etc.).
3. Provide assessment and linkage of clients to services.
4. Establish and facilitate curriculum in groups.
5. Program development
6. Evaluation of Client progress and DRC programming
7. Participate in trainings, conferences, and workshops.
8. Transport Clients

Pay: $38,000.00 - $45,000.00 per year
