We are seeking candidates for our Licesnsed Practical Nurse (LPN) positions, with full-time, part-time, and per diem options available.
The LPN supports individuals with intellectual and developmental disabilities by providing each individual with exceptional nursing support with special medical needs in order for the individual to experience a great quality of life at home and in the community.
On an on-going basis, the LPN monitors the individuals' physical and medical conditions, assures that the environment and activities are designed or adjusted to best meet special needs, administers medications and treatments as ordered by prescribers, and provides specialized nursing interventions as ordered. The LPN reports to the RN and additionally maintains clear communication with group home supervisors, staff teams, and families and guardians of the individuals.
Responsibilities of the LPN include but are not limited to:
- Assist individuals with disabilities to develop and maintain a high quality of life by supporting specialized needs for nursing intervention and support
- Plan and provide a daily schedule of activities, events, and experiences for individuals with medical needs
- Collaborate with other healthcare professionals to ensure comprehensive care for individuals with disabilities
- Provide emotional support and education to patients and their families
- Assist with the coordination of care, including scheduling appointments and arranging referrals
- Arrange for professional evaluations as needed for assistive devices, dietary needs, and other medically related needs as needed
- Provide assistance as needed with activities of daily living including personal care, and lifting and transfers using equipment such as a hoyer lift if applicable
- Administer medication and treatments as ordered
- Complete timely and accurate documentation
- Maintain medical files in conjunction with the program supervisors

To be successful in this role, you should have the following qualifications:
- Valid New Jersey LPN License
- Driver's License with fewer than four points
- Previous experience supporting people with developmental disabilities or a willgness to learn
- Knowlege of basic nursing procedures
- Excellent communication skills and ability to work well within a multidisciplinary team
