Supportive Behavioral Resources, Inc. is an agency located in Northeast Philadelphia that provides services to adults with Intellectual Disabilities and Autism. SBR is looking for a motivated professional to create and track programming for the individuals that we support, while sharing the mission of SBR, which involves enhancing the lives of individuals living with Intellectual Disabilities and Autism through Respect, Care, and Knowledge.
In-office work schedule: Monday through Thursday 9:00am-5:00pm, Friday 9:00am-3:00pm.
Position Overview
This position adheres to all mandated 6400/6100 regulations, agency policies and procedures while providing oversight of the quality of services provided to the individuals that are supported in the Home-Based Services Program.
Job Responsibilities
- Follows the mandates of the 6400 and 6100 ODP Regulations
- Creates goals that match the outcomes in ISPs for Home Based Services. This may include goals in the areas of activities of daily living, socialization, and recreation.
- Ensures all objectives and goals are being implemented as identified in ISP
- Conducts quarterly reviews of all program goals as identified in the ISP
- Runs daily Therap data reports for HBS and follows up regarding questions or concerns
- Reviews Therap daily to ensure that all required documentation (ISP Data, GERs, etc.) is completed
- Ensures that all waiver requirements are completed and accurate in each Individual ISP
- Ensures that reportable incidents are reported to the state through the EIM system
- Conducts investigations as assigned
- Follows up on all EIM Reports to ensure they are completed in a timely manner
- Creates Quarterly Incident Management Reports to be reviewed with HRT
- Updates agency Incident Management Policy as necessary
- Works closely with the HBS Manager to fill open shifts for call-outs, PTO requests and vacant positions
- Follows up with DSPs when there are concerns regarding Clocking In and Out
- Works with DSPs to complete the 24 hours of mandated training
- Follows up with DSPs when there are concerns regarding Clocking In and Out
- May work an occasional shift if needed by the department
Job Requirements
- Associate's degree; Bachelor's preferred in human services or related field
- 3+ years experience working in the Human Services Field
- Proficient and Professional Writing and Communication Skills
- Use of web-based data systems, Excel, and Microsoft Suite

- Attends/obtains required training as mandated by the regulation entities and SBR.

- Cooperates with investigations, audits, or surveys done internally or by external entities.
- Effectively communicates (verbally and in writing) in a concise and timely manner, ensuring the privacy of others and using respectful person-centered language.
- Documents interactions with employees, families, individuals, and external support entities using the approved method: Bamboo Note, Therap, and agency emails etc.
- Maintains professionalism at all times and promotes a positive work environment.
- Provides education to community members regarding the organization and people with disabilities, in partnership with advocacy groups and organizational or community efforts.
- Attends and actively contributes to SBR activities, including planning and development of social activities, meetings, and leadership opportunities.
- Advocates for those we support, their families, and the DSPs at local and federal levels.
- Adheres to all policies and procedures regarding HIPAA and Confidentiality.
- Performs all other duties as assigned.
- Participates in committees and task forces to support agency initiatives and to be able to communicate to staff the topics of importance.
