Summary: Assists clients in the Medication Assisted Treatment (MAT) Program with connection to substance abuse services as specified in the Individual Program Plan, Group Program Plan, Treatment Plan or Treatment Strategies or as an emergency requires. This may include assessment, service planning, linkage/referral, advocacy, crisis response planning, service plan evaluations, travel and reporting services.
Essential Duties and Responsibilities include the following; other duties may be assigned to meet business needs.

Acts as Westbrook’s primary contact with consumer(s) and as a MAT liaison.

Ensures a treatment plan is developed for each consumer and reviewed as required.

Advocates for and assists the consumer in obtaining needed services.

Provides supportive counseling services within scope of training and competencies.

Provides crisis assistance as needed.

Creates awareness by disseminating information to communities about substance abuse.

Conducts town hall meetings, gap analyses of existing services, and develops a strategic plan for MAT service.

Assists with Continuous Quality Assurance and participates in Utilization Management.

Complies with federal, state and company policies, procedures and regulations.

Maintains confidentiality of all consumer information and comply with HIPAA regulations

Demonstrates knowledge of and supports Westbrook Health Services mission, vision, values, policies and procedures, operating instructions, confidentiality standards and code of ethical behavior.

Must have valid driver’s license and Automobile Insurance
Education and/or Experience: Bachelor’s Degree in human services field as required by disability specific requirements, prior computer/PC experience using computerized Medical Records, telemedicine and scheduling systems is preferred, but not required. Incumbent must also possess basic math skills. Must pass Criminal Investigation Background and DHHR background check as well as post offer drug screen.
Physical Requirements: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Mobility and ability to sit for long periods use a standard computer keyboard, bend, reach and lifts up to 10 pounds are required in this position.
