Responsibilities:
The Licensed Professional Nurse/Licensed Practical Nurse provides intermittent skilled nursing services to patients in the home setting according to the patient’s plan of care. As a Licensed Professional Nurse/Licensed Practical Nurse you will:

Provide intermittent Skilled Nursing services including assessment, evaluation, procedures, teaching and training activities, including activities of daily living as outlined in the patient Plan of Care

Submit completed skilled nursing visit notes as requested by Clinical Supervisor

Participate in discharge planning process per Medicare Criteria

Attend staff meetings, team conferences and educational in-services as required

Be responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care
Qualifications:
Qualifications
- A current unencumbered State Professional Nurse License

Active CPR Certification
-
Must maintain a valid driver's license, reliable transportation, and good driving record

The ability to make sound professional clinical judgment

The ability to assess and document patient needs and meet those needs
-
Proficiency in Microsoft Office
