Help a child achieve a permanent home!
Position is generally Monday through Friday 9am to 5pm, which occasional evenings. No weekends!
QUALIFICATIONS: Bachelor's degree in social work, or another related human service field. CWEL preferred but not required. Ability to engage with families. Must be proficient in the use of a computer to complete all documentation required to complete the duties below. Experience working with children with special needs preferred, as well as experience or training in Reunification, Adoption and Sub-Guardianship. Must have a valid Illinois Driver's License and an ability to pass a background check and be able to pass State of Illinois written examination on Licensing Standards for Foster Family Homes (402), the Child Care Act.
Position Overview: Completing very lengthy, detailed adoption reports (could be 20 pages).
Summary of duties:
1. Provides consultation regarding permanency issues; identifies and removes barriers to permanency, working to secure appropriate placements for children; monitors and takes action to complete permanencies with children waiting for reunification or adoption/subsidized guardianship.
2. Assists the worker with the facilitates of community resources to prepare the home of parent.
3. Assists the worker with post adoption, Guardianship connection to the DCFS unit prior to finalization.
4. Works to secure additional service resources in a timely manner to expedite permanency.
5. Assists the worker to complete required documentation and home study to begin the Adoption or Sub Guardianship subsidy process.
6. Submits, monitors, and follows up with the needs of the subsidy packet pending DCFS approval.
7. Supports the worker with completion of request for out of state placement.
8. Provides linkages between caseworkers, recruitment resource specialists, adoptions staff and community providers to locate an adoptive home.
DIRECT SERVICE
A. Provide transportation for clients as directed by Foster Care Supervisor
B. Staff all cases with the clients’ foster care worker and Foster Care Supervisor
D. Check voice mail on working days and return messages in appropriate time frame.
E. Contact parents and/or foster parents regarding appointments or visits.
F. Contact other necessary parties for transports or visits.
II. OTHER DUTIES
· Actively participates in YSBIV’s Continuous Quality Improvement program
· Checks emails on a daily basis.
· Keeps outlook calendar updated on a regular basis and shares with supervisor
· Comply with rules established for the position.
· Submit service tickets and visitation records to supervisor on a weekly basis.
· Attend agency meetings, trainings, events and supervisory meetings as directed by Foster Care Supervisor

Pay: $45,000.00 per year
