Job Overview:
We are seeking a dedicated Registered Nurse to join our healthcare team. Service Supervisor’s assess patients’ home environments and instruct patients and their families. They must be able to work independently and may supervise CNA’s/PCA's in-home settings. Service Supervisor’s travel to patients’ homes, assisted/independent living facilities and other sites. Service Supervisor’s may spend considerable time walking and standing.
Assess patients and associated needs, develops POC for each patient. Service Supervisors must be available both by phone and in person for the hours the agency provides care to clients.
Qualifications:
Bachelor’s Degree and State of North Carolina Registered Nurse License
· At least One year of Home Care experience.
· Knowledge of CPR and emergency medical procedures
· Knowledge of drugs and their indications, contraindications, dosing, side effects and proper administration
· Knowledge of patient evaluation procedures.
· Ability to react calmly and effectively in emergency situations
· Ability to maintain quality, safety, and/or infection control standards
· Knowledge of accreditation and certification requirements and standards
· Ability to communicate effectively, both orally and in writing
· Knowledge of patient care charts and patient histories
Duties and Responsibilities:
1) Visit new client’s home to perform physical and environmental assessments, conduct the initial assessment for in-home aide clients and develop Plan of Care with client and family members. Complete all other documents required to provide in-home service to client. Orient in-home aide to POC as needed.
2) Visit clients within 75-90 days to perform aide supervision. Review client status and document any changes to physical assessment, environmental assessment of POC.
3) Verify competency of in-home caregiver.
4) Instruct in-home aide as needed to provide education for unfamiliar tasks
i.e. Hoyer lift operation
5) Visit client when necessary to address concerns/issues
6) Service Supervisor’s must be able to work independently and will supervise CNAs/PCA's and caregivers.
7) Performs miscellaneous job-related duties as assigned.
8) Must be available both by phone and in person for the hours the agency provides care to clients.

Pay: $33.66 - $40.54 per hour
Expected hours: 12 – 15 per week
