Position Overview:
The ALP RN will assist with ensuring the health and well-being of our residents by being responsible for the overall quality of care assessment, planning, documentation and provision of services to residents.
Essential Duties & Responsibilities:
- Evaluate resident’s needs, condition and care, and initiates and develop a comprehensive plan of care for individual residents. Knowledge of resident condition at all times.
- Assess resident on admission and obtain medical history, checks physician’s orders, transfer and pre-admission information. Review resident’s medical and social history. Conduct re-admission assessments for residents in area hospitals when necessary.
- Assist case manager with residents and resident representative in making application for and maintaining income entitlement and public benefits.
- Ensure proper preparation of PRI documentation and PRI forms and UAS used for resident care and reimbursement purposes (if applicable).
- Obtain emergency treatment when required and notify physician of emergency and take and carry out physician’s orders.
- Schedule appointments and arrange for transportation of resident to other facilities, as necessary.
- Assume ultimate responsibility for completeness of all care documentation; review and update care plans as needed, maintain medical records, perform charting duties and complete required medical record forms.
- Perform and/or supervise and assist with resident care according to policy and procedure.
- Make rounds of all floors on a periodic basis to assess care provided to residents.
- Handle problems and/or concerns expressed by residents, responsible parties, or family members regarding the care of resident.
- Responsible for completing and reporting to appropriate agencies any incident report forms for any accident or incident that may occur involving resident, staff or visitor.
- Resolve and monitor problems related to medication delivery services.
- Assist in arranging for any transfer of residents and complete required forms to do so. Assume ultimate responsibility for ensuring that medical records are transferred appropriately and in a timely manner when resident is transferred or discharged.
- Once the resident no longer meets the retention criteria, assist case manager with resident and resident representative and/or family member in the discharge process.
- Attends facility meetings, staff meetings and required or preferred in-services.
- Being on 24-hour call for emergencies and possibly filling in for staff in emergencies.
- Able to effectively communicate with residents, staff, families, physicians and outside agencies.
- Knowledge and familiarity with established policy and procedure which include but are not limited to Fire and Disaster Plan, Blood Borne Pathogens, Infection Control, Resident Rights, confidentiality and staff assignments.
- Promote high standards of care for residents and for compliance with all state and federal requirements and laws.
- Ensure residents’ rights are protected and personal preferences are accommodated, and that residents are treated by care staff in a dignified, respectful and caring manner.
- Ensure care procedures are in compliance with all state, federal and OSHA regulations.
- Perform all other responsibilities assigned by the Administrator and/or Administrator of Home Care & Community Services.
Basic Duties & Responsibilities:
- Communicates with referring health care team, including condition, goals, and changes in care.
- Assist in physician rounds.
- Interpret ALP and facility standards to care staff.
- Maintain and monitor infection control policies and procedures throughout the facility and interpret infection control program to all staff/ present information regarding infection control at new employee orientation.
- Assists with hands on duties as needed/necessary.
- Endorses and complies with the vision, objectives, procedures, policies of Bethany Village.
- Maintain resident confidentiality.
- All other duties as assigned.
Education Required:
Experience Required:
- Previous experience in various types of positions, including experience in dementia, geriatrics, human services, supervisory and/or administrative experience.
Hazards:

- Disinfectant and cleaning chemicals
- MSDS sheets are available for all chemicals used.
Equipment Used:
- Oxygen equipment and all other generally required nursing equipment.
- Computer, photocopier, fax machine, and general office equipment.
Working Conditions:
- Indoor Assisted Living Facility.
- Weekends and Holidays may be required as needed.
- May be required to work past regular shift, or off –shifts as necessary.
- Ability to participate in facility on-call program which may include working at facility during off hours.
- Exposure to emotional and psychological stresses associated with the care of elderly residents.
- Working conditions for this position may include the following: inside, odors, adequate lighting, working with/around others, working alone.
Physical Demands:
- Some heavy lifting required at times.
- Pushing wheelchairs.
- Constant walking, standing, turning, crouching, pushing wheelchairs & carts, sitting, talking, hearing, seeing, depth perception, and concentration are required for this position.
- The ability to concentrate on more than on job at a time, and the flexibility to handle multiple tasks and prioritize as needed are essential.
Personal Protective Equipment & Uniform Requirements:
- Business casual attire.
- Appropriate Personal Protective Equipment when performing personal care, injections, dealing with blood or body spills, and working with chemicals (cleaning agents).
