This Case Aid (CA) position holder assists patients, including scheduling transportation, registration, home visits, cancelling and setting Patient Assistant Programs and coordinating auxiliary services on behalf of patients. As well as provides support to case coordination for and behavioral health services to People living with HIV/AIDS (PLWHA). The holder of this position will give out-of-care clients individualized information and referrals to connect them to ambulatory outpatient medical care and other core medical services. Ryan White HIV/AIDS Program (RWHAP) Part D grant recipients provide outpatient family-centered primary and specialty medical care and support services. These services help low-income women, infants, children, and youth with HIV.
Maintains a regular and predictable work schedule. Accept incoming phone calls from patients regarding Social Service services and/or respond via work queue tasks within EMR · Conduct follow-up phone calls and no-show clinic appointments · Assist patients with scheduling appointments and setting transportation. · Coordinates file retention and storage efforts · Complete registration and request documentation. · Communicates with others (internally and externally) to provide, exchange, or verify information, answer questions, and address issues of clients. · Accomplishes service planning and coordinates with health providers to ensure the patient receives the correct level of care. · Participates in multidisciplinary case staffing to ensure coordination of care and high-level services provided to health center patients. · Familiarity with HIV/AIDS services and treatment, homeless services, and other community resources. · Objectivity and self-discipline to avoid emotional involvement in frequently emotional and high-charged situations. · Keeps abreast of new knowledge and techniques related to case management and new medical treatment modalities as they might affect patients' social adjustment or lifestyle via literature, professional settings, and staff development activities. · Documentation of services and referrals provided in the client record and required database(s), including an entry in EMR, EPIC, TCT, etc. · Performs data collection for quality management/assurance activities. · Assists with compiling monthly, quarterly, and yearly surveys. Participate in monthly calls and events from TRG.

Schedule: Full-Time, Mon-Fri (In person at the clinic – home visit and 1 day remote)
Position Key Accountabilities:
1. Performs initial intake interviews with patients and/or their families. Screens clients and assesses client’s needs.
2. Provides clients with orientation to the campus. Explains procedures and answers basic questions about the program.
3. Facilitates the completion of medical evaluation and a psychological or psychiatric evaluation for prospective clients.
4. Facilitates the completion of all documents required for admission, including documents for on-site schooling.
5. Facilitates transfer of clients to campus including transportation needs.
6. Records data related to relocation of clients/research subjects and activities in which individuals participate.
7. Codes and keys data into data files and maintains files in an orderly manner.
8. Coordinates with referral agencies/organizations for admission of prospective clients providing them with accurate and up-to-date information.
9. Identifies and develops additional sources of clients.
10. Maintains positive relationships with existing referral sources.
11. Performs other duties as assigned.

Minimum Education:
High school diploma or equivalent.
Minimum Experience:
One year of relevant work experience in a setting related to public contact, health care, or social service.
Physical Requirements:
Exerts up to 100 pounds of force occasionally and/or up to 50 pounds frequently and/or up to 20 pounds constantly to move objects.
