Care Navigator
JOB TITLE: Care Navigator
REPORTS TO: Director of Operations and Lead Care Navigator
SUMMARY:
The Care Navigator is responsible for driving initiatives, improving efficiency of the RTHM Medical Clinic. The primary focus will be supporting the providers at RTHM. This role reports to the Lead Care Navigator and the Director of Operations.
ESSENTIAL DUTIES, RESPONSIBILITIES AND EXPECTATIONS include the following as well as other duties and responsibilities that may be assigned.
Required Qualifications:
- Certified Medical Assistant
- Has worked in a remote setting.
- Experience using medical EHR/EMR platforms.
- Able to manage tasks, referrals, orders, lab lookup, and patient scheduling.
- Ability to prep charts and update medications.
- Engage with patients through phones, messaging, emails, care nav appts.
- Maintaining thorough, timely communication in EMR/EHR as well as Slack and email.
- Demonstrate proficiency in ordering and reporting; third party platforms.
- Delegation of tasks when appropriate to other team members.
- Able to follow SOPs correctly for typical job functions.
- Demonstrates consistency in time management in all above areas.
Preferred Qualifications:
- Experience with Cerbo EHR
- Has worked with Google Suite
- Experience with Slack
- Has worked at a Complex Illness Clinic

LANGUAGE SKILLS:
- Ability to read, analyze, and interpret general business periodicals, professional journals, and governmental regulations. Ability to write reports, business correspondence, and procedures with conformance to the prescribed style and format. Ability to effectively present information and respond to questions from government entities, employees, managers, clients, and the general public.
REASONING ABILITY:
- Ability to define and solve practical problems and deal with a variety of concrete variables, occasionally in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

- Must have excellent experience with administrative matters, i.e., insurance and reports.
ENVIRONMENT:
- Approximately 100% of the time performing job duties is spent indoors, within a home office environment.
- Approximately 90% of the time is spent on the computer.
PHYSICAL DEMANDS:
- Frequently (10%): sit, reach with hands and arms; climb or balance; stand and walk; grasp with hands and fingers; and lift up to 20 lbs
- Continuously (70-100%): use hands to finger, handle or feel objects, tools, or controls; see (including close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus), hear and speak; key (i.e., computer, calculator, telephone)
