ACE Program Coordinator (hybrid)

Tandigm Health is seeking a dedicated Program Coordinator to support our Hierarchical Condition Category (HCC) Programs. In this role, you will provide essential operational and administrative assistance to ensure the success of our Accurate Coding & Education (ACE) team. This role involves supporting day-to-day operations of programs designed to engage, enable, and empower our network of primary care physicians, helping them achieve accurate coding and documentation and meet patient quality of care goals.
The successful candidate will collaborate extensively with the ACE Medical Director, VP of Risk Accuracy, chart reviewers, coders, and practice consultants. This position requires cross-functional work as assigned by management on various company committees, including Financial Incentives, ACE Specialist Alliances, and Learning Management System education implementation. Additionally, you will engage with colleagues across Network Solutions, High Value Networks, Marketing & Communications, and other internal departments. Effective communication with external stakeholders, such as physician practice office staff, is also a key aspect of this role.
Essential Functions
- Collaborates with the ACE Team with the implementation of coding and documentation accuracy processes and clinical chart review programs.
- Participates in Financial Incentives and Operations quality control and quality assurance activities.
- Gathers and presents requirements and performs user acceptance testing on ACE operational program enhancements and requests in collaboration with IS&T.
- Creates and organizes educational materials, resources, presentations, and job aids.
- Acts as the ACE subject matter expert for the Tandigm Learning Management System platform and ACE Computer-Based Trainings and subsequent practice payment processes.
- Provides project management organization and support for ACE initiatives, as needed.
- Communicates and tracks ACE Key Performance Indicator and Workforce Management metrics.
- Acts as a resource expert to other team members, maintaining an understanding of Tandigm’s software, systems, processes, online resources, and CV completion platform, resolving issues, responding to requests in a timely and efficient manner, and providing education and training, as needed.
- Monitors and assists with accurate coding and network documentation audit processes and chart review activities as needed.
- Collaborates with practices and internal stakeholders on operational communications with the highest level of professionalism and accountability.
- Creates operational communications and facilitates exchange of information via e-mail and telephone.
- Coordinates meetings with external stakeholders; prepares meeting materials.

- Uses, protects, and discloses Tandigm’s patients’ protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards and protects Tandigm assets from unauthorized access, disclosure, modification, destruction or interference.
- Adheres to and responsible to abide by all information security policies, processes, and/or activities.
- Reports security events or potential events or other security risks to the organization to management.
- Performs additional duties as assigned.
Required Qualifications:
Education
- High school diploma, GED or equivalent (minimum)
- Associate’s or Bachelor's Degree (preferred)
Experience
- Minimum 2 years of related work experience, with at least 3 years of experience in a customer-oriented, project coordination role.
- Demonstrated written and oral communication skills.
- Proven ability to support multiple projects in an organized fashion with attention to detail.
