The Recovery Navigator (aka Support Navigator) is a high demand position that provides holistic, strength based, harm reduction case-management services specializing in substance use: education, prevention, maintenance, and recovery.
The Recovery Navigator is responsible for delivering various resources, which provide optimal quality care and consistency to residents living on-site. The Recovery Navigator facilitates groups as per the support model standards, develops recovery plans for resident’s who have received write ups for drugs or paraphernalia. The Recovery Navigator works closely with the exit team by creating a recovery centered exit plan for residents being exited from the program due to drug violations.
Recovery Case Management
· Initiate personal contact with new residents to introduce support model
· Research, coordinate, develop and facilitate groups per support model standards
· Develop, implement, and monitor individualized recovery plan for each resident requesting assistance using measurable goals and objectives by obtaining information from the intake assessment, medical history and physical screening, family assessment and psychosocial assessment
· Meet bi-weekly with residents expressing the desire to work on their recovery to provide support in the recovery process
· Develop recovery plans for resident’s who receive drug violation write ups for drugs or paraphernalia
· Create recovery centered exit plan for residents being exited from the program due to drug violations
· Provide appropriate recovery model referrals and link residents with treatment services
Record Keeping
· Maintain proper documentation including: goals, progress notes, monthly statistics, and client files
· Maintain resident tracker up to date
· Enter case notes within 24 hours into internal and external data base
· Exiting residents from internal database for accurate recording keeping
· Practice consistent communication electronic and verbal with Program Coordinator related to model standards and resident issues, concerns, progress. This includes keeping PC informed of services provided other service agencies on-site and/or services that resident is linked to
Miscellaneous Duties
· Participate in Homeless Service staff meetings and trainings
· Participate in Social Service and Community meetings such as the Recovery Collaborative
· Participate in resident meetings and huddles
· Participate in professional development trainings
· Participate in ALL site staff meetings, Navigator meetings, case conferencing meetings
· Utilize conflict management, de-escalation, and crisis intervention techniques in emergency situations
· Assist in maintaining positive working relationships with community partnerships
· Assist the Program Coordinator as requested
Other duties as assigned
Working Conditions:
Ability to walk, stand, bend, squat, climb, kneel and twist on an intermittent or sometimes continuous basis. Ability to grasp, push, pull objects such as files, file cabinet drawers, and reach overhead. Ability to operate computer, fax, and telephone. Ability to lift up to 40 lbs.
Minimum Qualifications:
- By virtue of its direct contact with, and representation to people who engage in Salvation Army programs, people who demonstrate an understanding of The Salvation Army’s Christian mission are required for this position
- Certified Alcohol Drug Counselor or equivalent (preferred)
- Minimum 1-year experience working within the homeless population, mentally & behaviorally ill (required)
- Minimum 1-year developing recovery case plans for individuals in recovery (required)
- Must be able to read, write and communicate in English
- Must be in good physical health and mental health, capable of meeting the demand of the position.
- Driving Test and clean MVR.
- Bilingual/Bi-literate English/Spanish preferred
- Must have ability to communicate effectively and remain positively engaged with co-workers, community members, and residents.
- Able to function in a fast-paced, frequently changing environment and be adaptable.
- Must have flexible availability.
- Must possess effective time management skills.
- Must be able to prioritize work and meet deadlines.
