In this pivotal role, you will spearhead the planning, organization, and execution of our Facility's Social Services Department. Your contributions will be generously rewarded with an annual salary range of $90,000 to $95,000.
Your Key Responsibilities Will Include:
- Conducting thorough patient assessments and orchestrating personalized care plans to aid patients and their families in acclimating to their new environment.
- Crafting, establishing, and executing comprehensive policies and objectives for the Social Services Department.
- Demonstrating your prowess in resolving escalated operational issues, which may necessitate seamless collaboration with other departments.
- Continuously assessing patients' evolving needs and equipping families with vital information regarding care planning.
- Engaging residents and their families in the design of social service programs whenever feasible.
- Offering innovative solutions to social and environmental challenges, including financial assistance, discharge planning, and liaising with community agencies when specialized support is required.
- Regularly interfacing with the administration, medical and nursing staff, as well as other pertinent stakeholders.
- Active participation in Facility Surveys conducted by authorized government agencies, as deemed necessary.
- Expertly interpreting the social, psychological, and emotional needs of residents and their families to facilitate seamless communication with the medical staff, attending physician, and other members of the resident care team.
