General Summary:
- Performs or delegates the initial and ongoing evaluation of patient needs within their scope of practice
- Routinely complete Start of Care of OASIS visits and may occasionally need to provide routine visits based on individual patient needs and branch expectations
- Ensures plan of care incorporates and guides appropriate teaching related to health maintenance, prevention and safety
- Coordinates available resources to manage care plan and ensures stated outcomes are achieved
- Periodically reassesses or delegates the reassessment of patient needs and revises care plan as necessary
- Assures appropriate care of patient is met through the start of care assessment completion.
- Promotes/coordinates communication between team members, attending physicians, appropriate administrative staff, referral sources and external case managers to ensure appropriateness of care
- coordination and communicates any necessary changes to the plan of care
- Ensures patient needs are identified and qualified caregivers are assigned to all cases within their scope of practice
- Participates in special projects and performs other duties as assigned
Registered Nurse (RN) Responsibilities:
- Follow a designated care plan in accordance with patient's needs
- Make home visits to clients in designated geographic territories
- Perform assigned duties, including administration of medication, wound care, treatments, and procedures
- Monitor clients' conditions; reporting changes to Clinical or Client Services Manager
- Follow up with, execute, and properly document doctors' orders
- Perform client assessments as necessary
- Case management and coordination
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet
Qualifications for a Registered Nurse (RN):
- A current license as a Registered Nurse in Nevada.
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Prior home care experience strongly preferred, but not required.
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
- Ability to travel to cases as assigned.
- Negative two step TB-(taken within one year)
- Must be current resident of Las Vegas, North Las Vegas, Henderson, or Boulder City.
