Responsibilities:
- Provide personal care services, including bathing, dressing, grooming, and toileting.
- Assist patients with mobility and transfers, ensuring their safety and comfort.
- Perform light housekeeping tasks to maintain a clean and safe environment for patients.
- Prepare meals and assist with feeding as needed.
- Monitor patients' conditions and report any changes to the supervising nurse or healthcare professional.
- Offer companionship and emotional support to patients and their families.
- Follow care plans and document services provided accurately and timely.
- Collaborate with healthcare team members to ensure coordinated and holistic care.
- Maintain confidentiality of patient information and adhere to all healthcare regulations.
Home Health & Hospice Aide Qualifications:
- Current Home Health Aide (HHA) or Certified Nursing Assistant (CNA) certification in the state of CA.
- 1-2 years of experience in home health or hospice care, preferably in a similar setting.
- BLS/CPR certification required.
- Valid CA Driver’s license and reliable transportation.
