Hospice Care Team is now hiring an Admission Nurse at our growing office in the south I-10 areas! The Admission Nurse will have the responsibility of delivering comprehensive patient care to all assigned patients, managing the Nursing Service Department in the absence of the DON, following all infection control procedures, participating in staff mentoring activities, and performing bereavement calls when appropriate. In addition, this position will be responsible for admitting new hospice patients and make skilled nursing visits as needed.

Responsibilities:
- Admitting new hospice patients as needed.
- Assesses the changes in patients’ conditions, documents the changes and reports them in a timely manner to the appropriate health care professionals including, but not limited to, the physician, nurses and social services team.
- Assists in ensuring compliance with all local, state and federal laws regarding licensure and certification of organization personnel and, maintains compliance to the Joint Commission Home Care standards.
Qualifications:
- Current, unrestricted RN license in Texas.
- Previous Hospice and/or Home Health experience
- Knowledge of regulatory agency requirements
