Position Summary:
Utilizes a systematic approach to nursing practice incorporating assessment, planning, nursing diagnosis, implementation and evaluation in accordance with the Nurse Practice Act, scope of practice, along with recognized professional standards and Health Partner and employer specific protocols and policy.
We are looking for an experienced RN to join our thriving Thrombosis team! This individual will support both our Thrombosis and Anticoagulation teams by partnering with care teams, coordinating patient care, providing education, and telephonic support to our patients. Candidates should have strong nurse practice skills, ability to effectively deliver education, competently prioritize tasks and possess a desire to bring out the best in others.
The Park Nicollet Thrombosis Clinic provides high-quality, patient-centered care to help patients manage conditions and situations that contribute to a high incidence of blood clotting. The Park Nicollet Anticoagulation Center (ACC) is a centralized nursing team specializing in anticoagulation medication management. Both of these partner with primary care teams, specialty care teams, and hospital pharmacists, to manage all areas of the patient’s anticoagulation and thrombosis care.
The fundamentals of this role include:
- Providing excellent care in a uniform and consistent method and using standing orders in an ambulatory setting
- An Emphasis on patient education the out-patient setting
- Ongoing patient education and empowerment to patients to be active participants in their care
- Multidisciplinary team approach to care, including functioning as a resource for other health care professional.
Work Schedule:
Mon - Fri 8:00am-5:00pm
Required Qualifications:
- Graduated from an accredited school of nursing
- Associate degree or diploma required
- Maintain a current Minnesota registered nurse’s licensure. WI licensure where applicable.
- BLS provider status in accordance with American Heart Association (AHA) guidelines
Preferred Qualifications:
- Bachelor’s Degree in Nursing
- Ambulatory Nursing Experience
- 1 year of prior experience in nursing preferred
