Position Overview:
The Health Services Coordinator is responsible for conducting comprehensive clinical health assessments of prospective and current residents and assisting the Health Services Director with the delivery of healthcare services within our organization. This is a pivotal role in our organization which requires administrative, managerial, and clinical skills. Operating in a fully on-site capacity, this position requires a high-level multi-tasking abilities and dedication. This role plays a critical role in ensuring that residents receive appropriate care and support tailored to their individual needs. The ideal candidate will have a compassionate demeanor, strong assessment skills, and a commitment to providing high-quality care to seniors.
General Responsibilities Overview:
Daily:
- Checking Group Me: Monitor Group Me communication platform for any updates, announcements, or urgent messages.
- August Health: Monitor Alert Charting system in August Health EHR system on a daily basis in collaboration with Resident Care Team
Weekly:
- Resident Issues: Assists with monitoring and tracking any resident skin issues and provide report to HSD/ED weekly.
- Track and report to the Health Department as necessary as well as to the state.
As Needed:
- Hospital Logs – Primary: Maintain hospital logs, documenting residents' hospitalizations and coordinating necessary follow-up care.
· Death/Incident Reports - Primary- Prepare and file state incident reports and death reports as required according to Title 22 regulations
· Email copy to HSD/ED for review prior to faxing to state.
· Upload signed copy to residents’ profile in EHR system.
· Place a copy in business office managers mailbox after faxing to the state to be scanned and uploaded.
- Hospice List: Ensure accuracy of hospice list, collaborating with HSD/ALW Coordinator to verify ALW/Private pay status.
- Insulin Reports: Keep track of residents on insulin and ensure regular Accu-Cheks are conducted.
- Assist with Insurance Inquiries and Assigning Physicians: Support marketing efforts by responding to insurance inquiries and facilitating physician assignments upon obtaining consent.
- Reassessments Responsible: Oversee reassessments process, communicating with RCD (Resident Care Director) upon completion to generate new service plans in August Health system.
- Incident Reports: Notify CCAs of incident reports for ALW residents, sending email notifications to alert ALW Coordinator of new incidents.
- Ongoing - Ensuring all 602s are up to date: Regularly review and update Form 602s (California Department of Social Services Community Care Licensing Division) to ensure compliance with state regulations.
Essential Functions:
Management:
- Supervise LVNs, particularly those involved in diabetic management, ensuring adherence to care plans and protocols.
- Serve as a liaison between physicians, residents, and families on health-related matters, facilitating effective communication and collaboration.
- Conduct training for LVNs and maintain comprehensive training records to ensure staff competence and compliance with regulations.
Resident Assessments:
- Perform thorough pre-admission clinical health assessments, including gathering medical history, medications, and treatment plans, to inform personalized care planning.
- Conduct readmission assessments post-hospitalization or sudden changes in condition, updating electronic health records (EHR) and communicating findings to the Resident Care Team.
- Provide ongoing assessment of residents' medical needs and coordinate care with external agencies and healthcare providers to ensure continuity of care.
Resident Care:
- Manage and communicate closely with outside agency services such as home health, hospice, and visiting clinicians.
- Ensure that changes in resident condition are evaluated, monitored, documented, and effective interventions are implemented.
- Collaboratively review Resident Health Records on a routine basis to ensure documentation meets Community standards and adequately addresses residents’ healthcare issues, including identified concerns, interventions, and outcomes.
- Oversee and manage any incidents or outbreaks among residents in the community, including tracking and reporting to the Health Department and the state as necessary.
- Exercise independent discretion in responding to resident emergencies, determining the need for and contacting emergency services, physicians, and families as appropriate.
- Document observations, concerns, changes in resident conditions, incidents, and/or accidents, ensuring that follow-up documentation occurs until the issue is resolved.
Additional Responsibilities:
- Act as a role model to other staff members in terms of professional appearance, communication, and conduct, demonstrating courtesy, gentleness, effective approach, and conversational techniques with residents.
- Assist the Health Services Director with other management duties as needed.
- Perform nursing tasks as necessary within the scope of practice, ensuring proper orders are in place and compliant with state law.
- Maintain an adequate inventory of healthcare supplies and equipment and follow appropriate Community protocols for purchasing.
- Ensure that all resident care is provided in a safe and effective manner, consistently maintaining resident dignity, choice, and respect.
- Maintain knowledge of current OSHA and state regulations, routinely monitoring Community compliance with regulations related to resident care and nursing functions.
· Attend daily, weekly Resident Care, and other requested meetings.
· Monitor group communication messaging app with staff.

Professional Qualifications/Minimum Eligibility Requirements:
- Experience working in Assisted Living Facility/Memory Care Facility
- Licensed Vocational Nurse/LPN OR RN Certification REQUIRED
- State license current and in good standing
- Previous supervisory and/or management experience in a related healthcare setting
- Ability to establish effective relationships with residents, family members and staff
- Strong documentation skills
- Ability to represent the Community in a positive and professional manner
- Experience in training and staff development
Working Conditions:
- Full-time position, requiring on-site presence during regular business hours.
- Some weekend work may be required.
- Salary position.
- Work is conducted in an office environment with standard office equipment and standard LED lighting.
