Spectrum Generations is a fast-growing non-profit organization that is committed to promoting and advancing the wellbeing and independence of older and disabled adults to live in their community of choice. The agency is one of five federally-designated Area Agencies on Aging in Maine. As the local AAA in Central Maine, the agency serves as a primary resource for information, guidance, education, and social, financial, and homecare programs and services for seniors, disabled adults and caregiver residents of Kennebec, Knox, Lincoln, Sagadahoc, Somerset and Waldo Counties plus the communities of Brunswick and Harpswell.

This is a full time, exempt position based in Kennebec and Somerset counties. This position can be partial work from home. Candidate must be a Maine resident. Residents from other Maine counties such as Androscoggin and Waldo are encouraged to apply.
We have a current need in Skowhegan, Madison, Pittsfield area (Somerset County) Waterville, Winslow and surrounding areas.
This is a great opportunity for a motivated and compassionate individual looking to further their case management experience with an energetic and positive team. The Community Case Manager coordinates the delivery of services for adults with developmental disabilities who may also have physical disabilities. This includes a comprehensive assessment of individual including identified support system, development of service plans, assisting the consumers and their families to link with the identified, available service provider(s), regularly monitoring on services plans and ongoing reassessment of consumers.
Duties include:
- Using a person centered approach to develop an individualized support plan reflecting the consumer’s personal vision for a desired life.
- Performing comprehensive assessment of consumer including health and safety, housing, vocational training/employment, physical health, income/finance, social/recreational/spiritual interests, psychological/psychiatric treatment and other needs.
- Facilitating consumer access to community resources/services
- Coordinating necessary and needed health and wellness activities including arranging for appointments and assisting as needed with acquisition of medical equipment and management of chronic conditions.
- Regularly meeting with consumer and continuously reassessing plans to assure satisfaction
Skills required:
- Ability to assess individuals with exceptional developmental disabilities as individuals and as members of families and their community.
- Ability to develop and implement comprehensive, consumer centered plans to achieve goals and implement the plan in a systematic manner
- Advocacy and negotiation skills.
- Ability to maintain effective written and verbal communications with providers and other health care professionals.
- Knowledge of biological, psychological, and social aspects of developmental delay; the impact of disabilities and illnesses on persons with developmental disabilities, human behavior and effects of psychological stresses.
- Knowledge of specific Intellectual Disabilities and associated signs and symptoms.
- Experience in treating the Intellectually Disabled or a directly related human services field and providing case management highly desired
Bachelor’s degree required in psychology, behavioral health, social work, special education, counseling, rehabilitation, nursing or closely related field. (Mental Health and Human Services/Education/Psychology/Social Services)
- Current Maine Driver’s License and current automobile insurance required
- Prior case management experience is preferred.
