As a Telehealth Medical Assistant, you’ll contribute to our success in the following ways:
- Ensures patients are appropriate for Remote Patient Monitoring by tracking adherence within RPM portal.
- Ensures patient engagement by monitoring engagement time within RPM portal.
- Reports any clinical issues to third party physician group as appropriate.
- Reports noncompliant patients to third party physician group for removal from program
- Provides reporting to SHH team while patient is on SHH services.
- Faxes monthly trends reports to primary care physician.
- Implements all available actions to prevent avoidable hospitalizations and ER visits.
- Provides positive, supportive communication to physicians, patients, families, visitors, and agency personnel.
- Actively works to connect patients with appropriate internal team members and third party team members based on patient needs and requests.
- Initiates and maintains collaborative relationships with patients, caregivers, insurance plans, and physicians.
- Participates in clinical record audits, utilization reviews, Peer Review or Quality Improvement programs, as appropriate.
- Other duties as assigned to support the changing needs of the business.
What is Required?
- Degree from a technical school or applicable skilled trades program
- 2+ years of experience as a Medican Assistant
- Current CMA or equivalent certification is required.
- Bilingual (able to read, write and speak) English and Spanish
- Valid driver’s license, reliable transportation and auto insurance in accordance with the laws of the state.
You will report to the Manager Patient Services.
