Our du Lac Assisted Living has two open Nurse Positions. We are looking for a Staff Development Nurse/Wellness Nurse to assist the RCD, Executive Director, and all du Lac Assisted Living Department Directors in the orientation and re-education of all facility staff, as well as a Full-Time Wellness Nurse to provide general nursing care to assigned residents including clinical intervention, assistance with medication administration, and monitor/supervise Personal Care Assistants (PCA) during assigned shifts.
Responsibilities:

responsible for the planning, conduction, and scheduling of in-services in accordance with current applicable federal, state, and local standards, guidelines, and regulations. (Staff Development Nurse)

Employee orientation and in-servicing will be ongoing. (Staff Development Nurse)

responsible to coordinate, track, monitor, and report on all du Lac facility required annual trainings.

Adhering to the resident Service Plan, provide general nursing care to assigned residents including clinical intervention, assistance with medication administration, and monitor/supervise Personal Care Assistants (PCA) during assigned shifts.

Responsible for PCA schedule (i.e. weekly PCA schedule, managing callouts while on-call, vacation scheduling, etc.).

Has the ability to be directly responsible for assessing, planning, providing and documenting nursing care for a designated group of residents in accordance with the resident’s service plan.

In the absence of the RCD, responsible to monitor and supervise unit shift activities, including oversight of PCAs.

Serves as leader and role model of customer service principles and consistently monitors and implements quality improvement efforts during assigned shifts.

Ensures that the unit functions according to accepted facility quality standards.

Cooperates with members of all departments within the facility.
Work Hours and Expectations:

Typical workday hours are Monday through Friday from 8:30 AM to 5:00PM. Work schedule includes occasional nights, evenings, or weekends for training facilitation purposes.

Participate regularly in a department on-call rotation; including may be required to report to work in the event of staffing shortages/call outs, emergencies, or as needed.

Position requires working every other weekend required and holidays as needed.

Required to work scheduled days regardless of inclement weather conditions.

Requirements:
- MA license as a Registered Nurse (R.N.) or Licensed Practical Nurse (LPN) required.

Minimum 5 years nurse experience required (preferably in Geriatrics).

A minimum of one year of assisted living or long-term care experience required. Minimum 2 years nursing supervisor experience required.

Assisted Living experience strongly preferred.

Minimum 2 years electronic health records experience required.

Intermediate level Microsoft Office (Word, Excel, and PowerPoint) preferred.

Excellent written and verbal communication skills required.
