This is a position working in an adult day habilitation program where you can gain valuable experience assisting special needs consumers.
Principal responsibilities include:
- Assist each consumer in skill development in areas such as independent living, adaptive behaviors, socialization, recreation and activities of daily living.
- Work with the consumers to maximize personal choice, rights, dignity and community inclusion.
- Strive toward the attainment of consumers’ goals.
- Implement all relevant support plans and, where needed, intervention.
- Supervise consumers in the community, using various experiences to reinforce the skills being taught in group session while ensuring their health, safety, and welfare.
- Ensure that consumers’ rights are preserved in all aspects of the program.
- Assist in the development and implementation of individual program plans.
- Coordinate services in accordance with the requirements of governmental and regulatory agencies.
- Maintains case records, incident reports, statistics and other related reports in an organized, timely, and accurate manner as per policy and procedure
- Works closely with clinicians to reinforce expectations for all consumers to ensure goal-directed and collaborative care
- Perform additional duties as assigned.
Education:
College degree preferred.
Skills and/or Experience Required:
Excellent interpersonal, verbal and written communication skills. Computer skills are mandatory. Must be creative, dynamic, enthusiastic, highly motivated, and have a positive attitude. Previous experience working with adults or families is required.
