ENGAGEMENT SPECIALIST
Interborough Developmental and Consultation Center is hiring Engagement Specialists for its CONNECT (Continuous Engagement between Community and Clinic Treatment) Program at its Williamsburg location. The goal for CONNECT is to look at individuals holistically in the context of their communities and ‘connect’ people to appropriate supports to improve their wellbeing. One of the unique features of this program is flexibility to meet the needs of the clients and the community. This position works under the general supervision of the Program Director.
QUALIFICATIONS:
● BA/BS Degree Required with 2 years of relevant experience
● Strong written and verbal skills
● Excellent computer skills are necessary, demonstrated ability to use word-processing and database programs. EHRs a plus.
● Knowledge of community resources and entitlements/benefits (SNAP, Medicaid/Medicare, etc)
● Exceptional organizational and interpersonal skills
● Able to work both independently and collaboratively
● Understanding of and experience working with individuals who were previously incarcerated and/or receiving intensive mobile treatment services
● Understanding of the multiple cultures in the communities we serve
● Able to adapt to changes based on client needs
● Bilingual Spanish encouraged to apply
This position requires significant fieldwork and flexibility to meet the needs of the clients.
RESPONSIBILITIES
● Conduct home visits, field visits, phone, and mail outreach.
● Engagement of client in Connect services by consistent coordination and collaboration with referral sources and client prior to clinic admission
● Works closely with the interdisciplinary care team including PCP, psychiatrist, therapist, residential services, substance abuse treatment program, etc.
● Participates in development and implementation of written individualized care plans
● Attends care coordination meetings to collaborate with out-reach staff and external support team members
● Collaborates with clients to determine their needs and discuss/advocate for needed services.
● Provides range of support services, including but not limited to, housing, benefits, legal, transportation/escort, vocational services, health and wellness, and support groups
● Conducts research to identify sources of support for client needs
● Participates in field engagement activities to provide awareness to community of Connect Program and services
● Actively engaged in community education on resources and supports as both a learner and teacher
● Additional responsibilities as assigned by the Program Director
● This position requires the candidate to be in the field at least 75% of the time and to take mass transit for travel.

SALARY RANGE: $52,000-$59,000 (Based on experience and licensure.)
