Overview:
The medical assistant plays a vital role coordinating care for patients with chronic conditions, collaborating with healthcare providers, and ensuring the delivery of high-quality, patient-centered services. This is an on-site position based in our Framingham, MA office. Not remote.
Responsibilities:
- Engage/enroll eligible patients, educate them on the value of our CCM services, and obtain verbal consent for services
- Gather and document medical history, chronic conditions, functional abilities, and psychosocial needs from various sources.
- Schedule appointments, referrals, and diagnostic tests as directed by healthcare providers and follow up on patient referrals and orders.
- Serve as the primary point of contact for patients, caregivers, and healthcare providers, facilitating communication and coordination of care.
- Advocate for patients' needs and preferences within the healthcare system, addressing any barriers to care or access to services.
- Monitor patients' health status, medication adherence, and progress towards treatment goals through regular check-ins and follow-up appointments.
- Follow up with patients between appointments to assess their progress, address any concerns or questions, and reinforce care plans.
- Proactively identify any changes or concerns in patients' health status and notify care team.
- Maintain accurate and up-to-date documentation of patient assessments, care plans, interventions, and outcomes in electronic health records (EHR) or other documentation systems.
- Prepare reports, summaries, and quality improvement data for review by healthcare leadership and regulatory agencies as required
- Communicate clearly and professionally with patients, families, and caregivers to ensure understanding of treatment plans and instructions.
Qualifications:
- Strong communication, interpersonal, and organizational skills.
- Knowledge of medical terminology, HIPAA regulations, and electronic health records (EHR) system.
- High school diploma or equivalent required
- Certification as a Medical Assistant (CMA) or Registered Medical Assistant (RMA) preferred.
- Previous experience in a healthcare setting, preferably in a role involving patient care and chronic disease management.
