We have an immediate need for a Registered Nurse for King County Public Health in Bellevue. This nursing position is responsible for providing comprehensive community health nursing services to prevent disease and promote health through the application of critical thinking and the ability to exercise independent judgment: This position will be part of a care team that works directly with and in conjunction with a medical provider managing and coordinating comprehensive patient care through care coordination, health education and disease management and prevention. It will include screening, assessing, planning, delivering and evaluating health interventions for individuals and families. Will provide support and assistance to medical providers and will coordinate direct patient care while facilitating patient flow in the clinic.

Duties to this Position:
- Coordinates care promoting clear communication among the care team and treating clinician
- Some Travel to patients home in a county vehicle
- Reviews charts of clients scheduled to be seen by a provider (medical or behavioral health) and ensures that the clients are following through with their appointments
- Arranges and tracks client’s appointments developing care plans; educating clients and coordinating and scheduling lab appointments; and assist in the overall management of high risk patients, particularly those with chronic diseases
- Helps with blood draws and vital signs when Medical Assistants are not available
- Contacts patients to report test results; complete referrals, change medications or care plan, notification of upcoming appointments (pre-visit phone calls), calls in prescriptions to the pharmacy, and follows up on Self-Management Goals in a timely manner
- Provides individual or group based education, counseling, or self- management goal setting sessions with clients
- Does routine and timely documentation of all core process work activities including, but not limited to, evidence-based chart reviews, patient calls & correspondence, and patient education activities.
- Develops interdisciplinary care plans and other case management tools, monitoring delivery of care
- Manages high risk patient care, including management of patients with multiple co-morbidity or high risk for readmission to a hospital setting
- Coordinates continuity of patient care with external healthcare organizations and facilities, including the hospital admission process, discharge, and referrals from the primary care physician or specialty care provider
- Participates in data collection and health outcome reports
- Promotes teamwork within the micro-system to improve patient access and the efficiency of the team and the site as a whole.
- Provides direct and indirect patient care.
- Assesses patients for urgency of need and prioritizes by acuity in a professional and friendly manner.
- Maintains accurate, appropriate and timely documentation in the medical records as needed and required by regulations.
- Participates in development and implementation of quality assurance procedures.
- Collaborates with clinical team regarding needs of clients/patients.
- Exhibits the ability to work with patients of various socio-economic backgrounds and temperaments; demonstrates tact, diplomacy and compassion; responds appropriately in emergency and stressful situations; acts as patient advocate.
- Ensures that all services are delivered in a culturally sensitive manner.
- Telephone calls and appointment consultations including blood pressure checks, injections and phlebotomy.
- Reads and provides appropriate documentation for TB testing.
- Performs dressing changes, wound care, and suture removal per provider orders, assists with procedures when needed.
- Provides patient education regarding both chronic and acute medical issues.
- Completes the RN Prenatal intake/ exam, maintains registry and provides case management as needed.
- Maintains Coumadin registry and titration.
Required Qualifications:
- Licensed as a Registered Nurse with the State of Washington
- Two years’ experience in health care.
- Clear Driving Record
- Ability to work collaboratively with clients and other staff.
