ALS Northwest is a dynamic non- profit organization providing support and resources for people living with Amyotrophic Lateral Sclerosis (ALS), their families, and caregivers living in the State of Oregon and the six counties of Southwest Washington.
We are seeking a full-time Social Worker/Care Services Coordinator for our Portland and SW Washington region. We are looking for a dedicated and engaging professional who is committed to education, advocacy and insuring people who are living with ALS and their families are well supported.
This position provides support, education and advocacy to families living with ALS and plays a vital role on various Multidisciplinary ALS clinics throughout the Portland Metro area including the Providence ALS Center. The Social Worker works directly with people with ALS and their families in their homes and in the community.
Major Responsibilities:
- Respond to client and family inquiries for information and support;
- Represent ALS Northwest on ALS Multidisciplinary Clinic and Center teams as required;
- Conduct home visits with people living with ALS and their families;
- Plan and facilitate ALS and Family Caregiver Support Groups;
- Coordinate access to care services programs and services;
- Advocate for individuals with ALS in accessing healthcare and community resources;
- Provide in-service ALS education to collaborative partners and community;
- Maintain professional knowledge of ALS, ALS Research and medical care and treatments;
- Engage with organizational staff as an active participant in providing services for people with ALS;
- Maintain client records and program data; perform administrative tasks as necessary;
- Other duties as required.
Minimum Qualifications:
- Master of Social Work from an accredited MSW program. Oregon MSW, CSWA, LMSW or LCSW ;
- Ability to assess both physical and psychosocial needs of people with ALS and their families based on knowledge of ALS and its ramifications;
- Knowledge of or demonstrated interest in learning about ALS, symptom management and treatment options for people with ALS
- Knowledge of family caregiver issues, resources and supports;
- Capacity to build a knowledge base of Medicare, Medicaid and other Insurance products and community healthcare systems and resources;
- Self-management, communication and time management skills; Proficient in Microsoft software;
- Possess and express a positive attitude toward people with ALS and people with disabilities;
- Demonstrated ability to work independently and establish and maintain professional relationships;
- Skill and experience in building rapport with clients and client interviewing;
- Professional commitment to client confidentiality;
Other Requirements:
- Local travel is required. Use of personal car for local travel with mileage reimbursement. Valid Oregon State Driver License and Car Insurance. Ability to drive rental vehicles.
- Hours- General working hours Monday through Friday 9am – 5pm – 40 hours per week with occasional weekend or after-hours events.
- Location- Headquarters (Portland) and Work from home available (Outreach into the community is required)
- Supervision- No supervisory requirements
- Reports to – Care Service Director
