Position Summary: The Healthcare Coordinator provides one-on-one health coordination and support to older adults residing within a community-based setting. The Healthcare Coordinator works closely with the RN Program Coordinator assisting clients with learning how to manage their healthcare as directed by medical providers, with the goal of working towards independence when possible. This role does not involve hands on, clinical nursing.
Principal Responsibilities:
- Schedule medical appointments
- Help identify and schedule available transportation if needed
- Provide advocacy and support during medical appointments
- Act as primary liaison to medical providers, families, advocates and other service providers
- Reconciles medication with provider and client
- Monitors outcomes of appointments and encourages completion of necessary medical follow up
- Explains, educates, and encourages client in developing a healthier lifestyle
- Collaborate with and refer to social work when psycho-social needs or services are identified
- Provide home visits to assist with health education
- Maintain accurate, timely documentation
- Other duties as assigned by supervisor
- Adhere to organization’s standards by completing annual mandatory trainings in a timely manner on topics including – but not limited to – Sexual Harassment Prevention Training, IT Security Awareness, Compliance, OSHA, HIPAA and any other training the organization may deem necessary in regard to regulatory compliance or good business practices.

- Adheres to the organization’s policy in regards to absenteeism and appearance and health and safety standards.
Compensation: $68,000 - $72,800 commensurate with experience.
Job Competencies & Minimum Qualifications:
- LPN License and New York State Certification required
- One year of experience working in the health care field preferred experience as or a combination of education and two years' experience in community healthcare setting.
- Knowledge and experience with geriatrics, intellectual and developmental disabilities preferred.
- Must be able to conduct home visits.
- Working knowledge of Microsoft Office Suite and other technology
- Excellent communication and listening skills
- Excellent customer service skills with a focus on treating clients with respect and dignity
- Able to multi-task and work independently with a great attention to detail
Working Conditions/Physical Demand:
- Business office environment with phone and computer use.
- Must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds.
- Employee will be expected to travel to client’s home via public transportation.
- This position is hybrid schedule with office/field work 3-4 times per week.
