Sunnyside Community Services (SCS) is a multiservice, community-based organization serving New York City with a focus on western Queens — an area that is home to a uniquely diverse, predominately immigrant population and many low-income families. Established as a small senior center in 1974, SCS began offering licensed home care services in 1979 and introduced youth services in 1983. Today we provide a full range of human services to 16,000 individuals each year including children, youth, adults, and seniors. Our mission is to provide a continuum of vital services and activities that enrich the lives of individuals of all ages.
Position Description
The overall responsibility is to lead a team of four case management supervisors, who in turn supervise 17 case managers, administrative assistant and provide oversight for case management intake. We are looking for someone with a strong gerontological social work background and program management background. The right candidate will have an ability to motivate others, resolve conflicts and advocate for the social work perspective while maintaining compliance with the New York City Department for the Aging contractual requirements of case management. Candidate will be an effective leader, able to establish trust through transparency with their team and lead with diplomacy.
The case management program provides social services in western Queens to over 1300 homebound older adults annually. Through coordination of home delivered meals, home care and connections to vital services including transportation, and benefits, the program works to help older adults remain living safely in their homes for as long as possible.
Responsibilities
· Direct a team of 23 professionals in the provision of case management services to homebound older adults.
· Provide weekly supervision and as needed coaching to four supervisors.
· Ensure that the department is operating in compliance with contractual obligations, governmental regulation and professional standards.
· Collaborate with Elder Justice, Caregiver, Geriatric Mental Health programs and SCS’s overall continuum of programs for older adults.
· Collaborate with other external stakeholders to ensure appropriate service provision.
· Monitor quality of care and compliance through accompanying home visits, case note review, and supervisory meetings.
· Lead effective, inspiring and inclusive team meetings.
· In collaboration with Associate Executive Director of Older Adult Services, develop strategies and work from a strength-based perspective to meet the needs of older adults to age in their home with dignity.
· Participate in educational training and conferences and ensure staff participate in trainings per contractual requirements.
· Be a clear and concise communicator to ensure the delivery of high-quality performance of staff.
· Prepare and submit reports to NYC Aging program officer, supervisor and as required.
· Develop staff structures and program deliverables in careful coordination with budgetary and programmatic framework. Be able to forecast trends and make corrections within a changing environment.
Qualifications
- Master’s in Social Work and at least five years of supervisory experience.
- LMSW with SIFI preferred.
- Strong alignment with the mission of Sunnyside Community Services.
- Strong interpersonal skills and able to motivate and develop staff.
- Experience in Case Management programming highly preferred.
- Ability to lead a diverse group of staff.
- Self-starter with strong organizational, judgment, planning, and time management skills.
- Demonstrated experience in managing a staff of 10 or more.
- Passion for advocating on behalf of older adults.
- The ability to track and report on data is essential.
- Experience with NYC Aging Peer Place and standards is a plus
