We are seeking a highly organized and detail-oriented MDS Coordinator to join our healthcare team in Riverhead,NY. The MDS Coordinator will play a crucial role in ensuring accurate and timely completion of Minimum Data Set (MDS) assessments, collaborating with interdisciplinary teams to enhance resident care plans, and maintaining compliance with regulatory requirements.
Responsibilities:
- MDS Assessment:
- Conduct comprehensive assessments of residents' medical, physical, and psychosocial conditions.
- Ensure accurate and timely completion of MDS assessments in accordance with state and federal regulations.
- Care Plan Development:
- Collaborate with the interdisciplinary team to develop individualized care plans based on MDS assessments.
- Monitor and update care plans as needed to reflect changes in residents' conditions.
- Quality Assurance:
- Maintain compliance with state and federal regulations related to MDS assessments.
- Participate in quality improvement initiatives to enhance resident care and outcomes.
- Interdisciplinary Collaboration:
- Work closely with nursing, therapy, and other departments to gather relevant information for MDS assessments.
- Facilitate communication and coordination among team members to ensure comprehensive resident care.
- Training and Education:
- Provide training to staff members on MDS processes and documentation requirements.
- Stay informed about updates to MDS regulations and implement necessary changes.
Qualifications:
- Licensed Registered Nurse (RN) in the state of New York.
- Minimum of 2 years of experience as an MDS Coordinator in a long-term care setting.
- Strong understanding of MDS processes, regulations, and reimbursement systems.
- Excellent communication and interpersonal skills.
- Detail-oriented with strong organizational and time-management abilities.
