Job description
We are looking for an Housing Assistance Case Manager to coordinate general administrative duties and data tracking.
Requirements:
- Associate's Degree in Social Sciences or related fields
- 1 Years Experience working in an office environment
- Proficient in all Microsoft Office applications, including Excel, Word, and Outlook
Responsibilities:
- Conduct office work and at various field locations using portable communication devices
- Utilize online request forms and reporting templates for efficient communication and data collection
- Create a resource package outlining community benefits available to program participants
- Ensure the availability of translation services to accommodate diverse program participants
- Coordinate case management and housing navigation services for each participant
- Prepare and distribute welcome packets for participants entering program
- Develop housing plans that allow for placement for eligible participants in temporary or permanent accommodation
- Arrange for transportation and storage facilities as needed
- Coordinate access to various community services and resources, facilitating a warm hand off after program completion
- Conduct weekly participant wellness checks on each participant enrolled in the program
- Assess and monitor participant progress while enrolled in program
- Undertake training in Trauma-Informed Care, Mental Health First Aid, Infection Control, common knowledge of communicable diseases, and program policies and procedures
- Conduct comprehensive assessments of participants to identify their needs during program enrollment
- Maintain accurate and up-to-date participant records, documenting assessments, interventions, and progress towards goals
- Perform other tasks and duties as assigned on various community health programs

Pay: $22.00 - $24.00 per hour
Expected hours: 40 per week
Schedule:
- Monday to Friday
- Weekends as needed
Ability to Relocate:
- San Diego, CA 92108: Relocate before starting work (Required)
