The Social Worker Assistant coordinates the continuum of patient care in accordance with the vision and goals of the Oak Valley Hospital District (OVHD). Ensures quality, cost-effective patient care through planning, communication, critical thinking and facilitation of the multi-disciplinary patient care team. Performs utilization review functions, optimizing patient discharge with Medicare/HMO/other insurance standards.

JOB REQUIREMENTS
- High school graduate or equivalent required
- Must be computer literate. Able to perform basic mathematical calculations.
- Bilingual (English/Spanish) preferred.
