Overall Function
Responsible for conducting eligibility intakes, developing service/treatment plans and providing ongoing case management services for families and individuals to help them improve their life outcomes.
Essential Job Responsibilities:
- Conducts a comprehensive assessment & intake eligibility of services for families and individuals.
- Develops service plans in conjunction with the family and individual based on assessments.
- Acts as an advocate in obtaining resources and connecting to services, including accompanying families and individuals as necessary.
- Maintains frequent contact with families and individuals to follow through with services and ongoing support.
- Acts as a mentor to families and individual in educating them in the area of academic and developmental resources, health, and parenting education.
- Completes required program documentation and reports, including case notes.
- Participates in outreach activities on an as-needed basis, e.g. outreach fairs, and presentations to community groups and agencies.
- Bi-lingual preferred
