Salary Compensation Range: $60,000-70,000
MAJOR FUNCTIONS:
The Senior Family Care Coordinator works with pregnant and post-partum persons who misuse drugs or have a substance use disorder in a variety of community-based and institutional service delivery settings as well as in the community-at-large. The major functions of this role are to provide screening, assessment, referrals to care and case management to the identified population.
The Senior Family Care Coordinator offers referral to substance use disorder prevention, treatment and recovery services, harm reduction services as well as collateral and supportive services. The Senior Family Care Coordinator provides counseling to assist with engagement in care.
SPECIFIC DUTIES & RESPONSIBILITIES:

Conduct screening for co-occurring substance use and mental health disorders.

Complete substance use disorder assessments and formulates psychosocial evaluations.

Provide education and assists in accessing addiction medications.

Conduct level of care determinations.

Deliver Screening, Brief Intervention and Referral to Treatment (SBIRT) services.

Provide referrals and care coordination for substance use disorder prevention, treatment, and recovery services.

Provide referrals and care coordination for healthcare, dentistry, psychiatry, rental and housing assistance, health insurance options, legal assistance, domestic violence victim’s services, vocational training, and remedial education.

Maintain effective lines of communication with Bronx and Northern Manhattan ACS offices and ACS-contracted child welfare agencies.

Provide opioid overdose reversal training and medication to program participants.

Deliver Motivational Interviewing to assist participants in planning and engaging in desired services.

Deliver Cognitive Behavioral Therapy to assist participants in developing resiliency and healthy coping responses.

Participate in inter-agency team meetings and service planning conferences.
-
Document services rendered in clinical case record.

Participate in quality improvement activities.
-
Participate in in-service training according to program requirements.
REQUIREMENTS: EDUCATION/TRAINING AND EXPERIENCE KNOWLEDGE, SKILLS AND ABILITIES:

Ability to deliver culturally competent services to a diverse client population in an urban setting.

Ability to engage professionally in inter-disciplinary settings.

Must be able to work a flexible schedule.

Bi-lingual Spanish/English fluency preferred.

Proficient with MS Office applications; ability to navigate and document in EHR required.

LMSW, LMHC, or LMFT required (or the ability to obtain within 6 months of hire) OR Advanced CASAS-AC and at least two years supervisory experience.
