TITLE: RNor LPN -PERSONAL CARE MANAGEMENT
POSITION SUMMARY:
The Registered Nurse or LPN participates in screening potential program participants in their homes and provides on-going monitoring and support to program participants.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
1. Be knowledgeable of and comply with all state, federal and agency regulations, policies and procedures pertaining to the PCM program and WestMass ElderCare’s mission.
2. Assesses the consumer’s physical and cognitive condition and resulting functional limitations to determine the consumer’s ability to benefit from personal care assistance services.
3. Provides an initial determination of the scope and type of personal care services to be provided to consumers. Accurately represents the consumer’s need for physical assistance with ADL’s and IADL’s.
4. Schedules and provides yearly re-evaluation of consumers according to timeliness set forth in regulations.
5. Completes the Mass Health application for PCA services including calculating units when requesting prior authorization.
6. Completes prior authorization form.
7. Conducts functional skills training with consumers and surrogates as necessary.
8. Provides quarterly visits as required by program regulations.
9. Personal Care Management Quality Standards
· Prior Authorization requests -0 Denials
· Prior Authorization requests - Modifications of more than a 25% change in original PA submission will be less than 10% of PA submissions
· 95% of chart audit standards for semi-annual record review
· 100% compliance with contract standards
· Consumer Satisfaction Survey – 95% satisfaction
10. Assists with monthly and bi-annual reports for the PCM Program in consultation with the Program Director.
11. Attends meetings as directed by the Program Director.
12. Participates actively in outreach programs to identify individuals who could appropriately be served by thePCM program.
OTHER RESPONSIBILITIES:
- Performs other related duties as requested and needed that are within the scope of the responsibilities of this position.
CERTIFICATIONS, LICENSES, REGISTRATIONS:
- Valid driver’s license
- Satisfactory driving record
- Satisfactory background check that includes CORI, SORI, SAMs GSA, Mass Health Exclusion List and OIG check
- Current, Massachusetts Registered Nurse license or LPN License
REQUIRED KNOWLEDGE. SKILLS AND ABILITIES:
- Has the ability and willingness to support the goals and policies of WestMass ElderCare.
- Diploma from an accredited school of nursing, B.S.N. (preferred).
- Valid license to practice as a registered nurse in the Commonwealth of MA.
An understanding and acceptance of the role of the Registered Nurse at WestMass ElderCare which includes:
- Assessment
- Referral to appropriate agency and/or service
- Consulting with Program Director and with other appropriate agency and non agency staff regarding the nursing assessment and the consumer’s needs.
- Evidence of continued professional growth, either through formal education and/or attendance at workshops, seminars, continuing education and in-service programs.
- Experience with working with individuals of all ages and with disabilities.
- Must be able to handle confidential data and information discreetly.
- Must have an ability and willingness to work in a changing environment.

- Must have computer skills including WORD and ability to work with Data program.
- Must be attentive to details and capable of accurate work.

- Availability for mutually agreed upon number of cases per month.
- Must provide personal and reliable transportation
The role of the Registered Nurse at WestMass ElderCare does not include:
- Making diagnosis
- Direct hands on care which includes but is not limited to treatment, preparing medications, etc.
Job Type: Full-time
Pay: $35.00 - $38.00 per hour
