The primary role of the Case Manager is to provide comprehensive strength-based case management services to FTS clients,including the successful transition into and maintenance of permanent housing.
- Monitor and conduct weekly case management meetings with clients on a one-on-one basis making any necessary recommendations for additional services
- Monitor client files to ensure quality, grant compliance, and all information is accurate and up to date
- Maintain client confidentiality in accordance with FTS policy and procedures as well as funding regulations
- Aid individuals and/or families struggling with permanent housing, social services, mental health, medical concerns, domestic violence, and parenting assistance (as needed)
- Locate and make appropriate recommendations for community resources and linkage to services for clients
- Evaluate client’s readiness to transition;including the formulation of service plans and goals, and aid clients in implementing service plans
- Attend internal and external trainings to stay abreast of changes
- Attend Multidisciplinary Case Planning and Supervision Meetings
- Maintain working knowledge of all FTS policy and procedures
- Perform duties as assigned by supervisor and/or Directors

Pay: $24.00 - $26.00 per hour
